1 材料和方法1.1 材料活检标本取自260例患者,年龄18岁~74岁,平均40.3岁,男女比例为1.36:1,其中内镜诊断为慢性胃炎194例,胃溃疡14例,十二指肠球部溃疡52例.所有患者均未接受抗 Hp 治疗,胃癌和胃结核未纳入本研究.抗 HLA-DR 单克隆抗体为丹麦 Dako 公司产品,ABC 试剂盒为美国 Vector 公司产品.1.2 方法每例患者均在胃窦采集胃粘膜组织3块,1块用作Hp 培养,1块用作快速尿素酶实验,另1块作病理学检查.快速尿素酶检查或 Hp 培养出现一项阳性即认为有 Hp 感染存在.免疫组化染色采用 ABC 过氧化物酶法.用正常鼠血清代替一抗进行染色,作为阴性对照.用磷酸缓冲液代替一抗进行染色,作为空白对照.胃粘膜组织活检标本经福尔马林固定、脱水后石蜡包埋,4μm厚连续切片,分别作 HE 染色和 ABC 免疫组化染色,应用普通光学显微镜观察,若观察到淋巴细胞聚集和清晰的生发中心即认为有淋巴滤泡的存在,用 x~2检验判定 Hp阳性组与 Hp 阴性组胃粘膜淋巴滤泡发生差异有无显著性.根据淋巴滤泡周围 HLA-DR 抗原阳性上皮细胞表达范围分别判定:无胃粘膜上皮细胞表达 HLA-DR 抗原(-),紧邻淋巴滤泡不超过一个腺体范围的上皮细胞表达 HLA-DR 抗原(+),距淋
