目的 探讨儿童Ⅰ型糖尿病与肠道病毒 (EV)感染 ,特别是柯萨奇B组病毒 (CVB)感染的关系。方法 采用RT PCR方法 ,对 45例糖尿病患儿血中EV RNA进行检测 ,用微量中和试验方法 ,对糖尿病患儿和正常小儿血清中抗CVB中和抗体水平进行测定 ,同时用ELISA方法 ,测定CVB特异性IgM。结果 (1)糖尿病组 45例患儿中EV RNA阳性者为 17例 ,(39% ) ,对照组 5 1名儿童中阳性者为 3名 (6 % )。 (2 )检测 35例糖尿病患儿CVB IgM阳性者为 14例 (40 % ) ,40名对照组儿童中阳性者2名 (5 % )。 (3) 4 0例糖尿病患儿的CVB中和抗体检出为 32例 (80 % ) ,其中抗体异常者为 18例(5 6 % ) ;对照组CVB中和抗体检出为 40例 (82 % ) ;CVB感染型别分别以CVBB4 ,CVBB3 为多。结论儿童Ⅰ型糖尿病发病与肠道病毒感染 ,特别是柯萨奇B组病毒感染有关。
Objective To investigate the relationship between childhood type 1 diabetes and enterovirus (EV) infection, especially Coxsackie virus B (CVB) infection. Methods RT-PCR was used to detect the level of EV RNA in blood of 45 children with diabetes and the level of anti-CVB neutralizing antibody in the serum of children with diabetes and normal children was measured by the method of micro-neutralization. The ELISA method was used to determine CVB-specific IgM. Results (1) Among the 45 children with diabetes mellitus, 17 were positive for EV RNA (39%), and 3 (6%) were positive for 51 children in the control group. (2) 14 cases (40%) were positive for CVB IgM in 35 diabetic patients and 2 (5%) were positive in 40 control children. (3) There were 32 cases (80%) of CVB neutralizing antibodies in 40 children with diabetes mellitus, among which 18 cases (56%) had abnormal antibodies and 40 cases (82 %); CVB infection type respectively CVBB4, CVBB3 as much. Conclusion The incidence of type 1 diabetes in children is related to enterovirus infection, especially Coxsackie B virus infection.
