目的探索研究肿瘤坏死因子-α(TNF-α)、环氧化酶-2(COX-2)、P53以及细胞角蛋白7(CK7)在Barrett食管(BE)中的表达情况,分析其变化的意义。方法 38例明确BE诊断患者,通过免疫组织化学对BE患者术后组织进行切片染色,将患者BE病变区域组织设为BE组(42例),将BE边缘食管炎部分组织作为食管炎组(27例),将BE病变部位外5 cm正常食管黏膜作为正常组(29例)。比较三组TNF-α、COX-2、P53及CK7的表达情况。结果正常组TNF-α阳性率为20.7%,食管炎组为92.6%,BE组为81.0%,正常组TNF-α阳性率低于食管炎组、BE组,差异具有统计学意义(P<0.05);食管炎组和BE组TNF-α阳性率比较差异无统计学意义(P>0.05)。正常组COX-2阳性率低于食管炎组、BE组,差异具有统计学意义(P<0.05);食管炎组和BE组COX-2阳性率比较差异无统计学意义(P>0.05)。正常组P53蛋白阳性率低于食管炎组、BE组,差异具有统计学意义(P<0.05);BE组P53蛋白阳性率高于食管炎组,差异具有统计学意义(P<0.05)。正常组、食管炎组CK7蛋白阳性率比较差异无统计学意义(P>0.05),BE组CK7蛋白阳性率均高于正常组和食管炎组,差异具有统计学意义(P<0.05)。结论 TNF-α、COX-2、P53及CK7与BE的发生具有密切联系,通过检测其表达量能够对BE诊断提供一定指导意义。
Objective To explore the expression of tumor necrosis factor-α (TNF-α), cyclooxygenase-2 (COX-2), P53 and cytokeratin 7 (CK7) in Barrett’s esophagus significance. Methods Thirty-eight patients with definite BE were enrolled in this study. Tissue sections of patients with BE were stained by immunohistochemistry. The BE lesions were divided into BE group (42 cases) and BE marginal esophagitis group as esophagitis group (27) Cases), the BE lesion 5 cm normal esophageal mucosa as the normal group (29 cases). The expression of TNF-α, COX-2, P53 and CK7 in three groups were compared. Results The positive rate of TNF-α was 20.7% in normal group, 92.6% in esophagitis group and 81.0% in BE group. The positive rate of TNF-α in normal group was lower than that in esophagitis group and BE group (P <0.05) ). There was no significant difference in the positive rate of TNF-α between esophagitis group and BE group (P> 0.05). The positive rate of COX-2 in normal group was lower than that in esophagitis group and BE group (P <0.05). The positive rate of COX-2 in esophagitis group and BE group had no statistical significance (P> 0.05). The positive rate of P53 protein in normal group was lower than that in esophagitis group and BE group (P <0.05). The positive rate of P53 protein in BE group was higher than that in esophagitis group, the difference was statistically significant (P <0.05). The positive rate of CK7 protein in normal group and esophagitis group was no significant difference (P> 0.05). The positive rate of CK7 protein in BE group was higher than that in normal group and esophagitis group (P <0.05). Conclusions TNF-α, COX-2, P53 and CK7 are closely associated with the occurrence of BE. The detection of TNF-α, COX-2, P53 and CK7 can be helpful for the diagnosis of BE.
