目的:观察慢性肺源性心脏病患者血小板参数变化与棘形红细胞的关系。方法:95例慢性肺源性心脏病患者,使用仪器法检测血小板计数、平均血小板体积、血小板分布宽度、血小板压积。同时用电镜观察静脉血液中的棘形红细胞,并做正常对照。结果:95例平均血小板体积、血小板分布宽度、血小板压积测定结果较正常对照组均明显增高(P<0.05),且棘形红细胞明显增多(P<0.01)。结论:棘形红细胞可使红细胞硬度增加,变形能力下降,引起肺动脉高压,缺氧和CO2潴留等一系列病理变化,使血小板过度消耗,骨髓代偿性生成大体积血小板增加,故平均血小板体积、血小板分布宽度增大,血小板压积增高。功能增强的血小板更易被缺氧和CO2潴留激活,而加重慢性肺源性心脏病的高凝状态。
Objective: To observe the relationship between platelet parameters and splenocytes in patients with chronic cor pulmonale. Methods: Ninety-five patients with chronic cor pulmonale were enrolled in this study. The platelet count, mean platelet volume, platelet distribution width and platelet pressure were detected by instrumental method. Electron microscopy was used to observe the spiny erythrocytes in the venous blood and to make normal control. Results: The average platelet volume, platelet distribution width and platelet pressure in 95 cases were significantly higher than those in normal control group (P <0.05), and the number of splenic erythrocytes was significantly increased (P <0.01). CONCLUSIONS: Erythrocytes can increase the hardness of erythrocytes, decrease the deformability, cause a series of pathological changes such as pulmonary hypertension, hypoxia and CO2 retention, which lead to over-consumption of platelets, increase of large-volume platelets compensated by bone marrow, so the average platelet volume, Increased width of platelet distribution, increased platelet pressure. Enhanced platelets are more likely to be activated by hypoxia and CO2 retention, aggravating the hypercoagulable state of chronic cor pulmonale.
