目的探讨CXC趋化因子配体10(CXCL10)与2型糖尿病早期肾病(DN)之间的关系,为DN的诊断、治疗及预后提供新的途径。方法选择2型糖尿病及2型糖尿病早期肾病患者各50例,分为2型糖尿病组和2型糖尿病早期肾病组,采用酶联免疫试验(ELISA)测定两组患者的血清CXCL10水平,并分析不同组别CXCL10水平的差异,以及CX-CL10与患者年龄、性别、病程、腰臀比、体质指数(BMI)、糖化血红蛋白(HbAlC)、空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、UA、尿白蛋白排泄率(UAER)等指标之间的关系。结果两组年龄、性别、腰臀比、BMI、FPG、TG、TC、LDL-C、HDL-C差异无统计学意义(P>0.05);两组病程、UAER、HbAlC及CXCL10水平差异有统计学意义(P<0.01),2型糖尿病早期肾病患者的病程、UAER、HbAlC及CXCL10水平均长于或高于2型糖尿病组。结论趋化因子CXCL10可能参与了DN的发生、发展过程,在DN的发病机制中起重要作用,临床上可作为诊断早期DN及判断DN进展的参考指标。
Objective To explore the relationship between CXC chemokine ligand 10 (CXCL10) and type 2 diabetic nephropathy (DN), and to provide a new approach for the diagnosis, treatment and prognosis of DN. Methods 50 patients with type 2 diabetes mellitus and 2 type 2 diabetic nephropathy were divided into two groups: type 2 diabetes mellitus group and type 2 diabetic nephropathy group. Serum levels of CXCL10 were determined by enzyme linked immunosorbent assay (ELISA) The difference of the level of CXCL10 between the two groups and the relationship between CX-CL10 and age, gender, course of disease, waist-hip ratio, body mass index (BMI), HbAlc, (TC), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), UA, urinary albumin excretion rate (UAER) and other indicators. Results There were no significant differences in age, sex, waist-hip ratio, BMI, FPG, TG, TC, LDL-C and HDL-C between the two groups (P0.01). The duration of UAER, HbA1C and CXCL10 in type 2 diabetic patients with early nephropathy was longer or higher than those in type 2 diabetic patients. Conclusion The chemokine CXCL10 may play an important role in the pathogenesis of DN, which may play an important role in the pathogenesis of DN. It may serve as a reference index for diagnosis of early DN and progression of DN in clinic.
