目的探讨根管治疗失败的原因及临床对策。方法对160例第一,二磨牙根管治疗失败的原因进行临床分析。结果术后疼痛肿胀10例,器械折断16例,髓腔穿孔及根管侧穿16例,根管欠充填及根管超充填78例,根管遗漏22例,根管台阶6例,牙折12例。结论根管治疗要掌握好根管预备技术,避免形成台阶,拍摄治疗前,治疗后的X线片,必要时可以放入扩大针来拍摄X线片,测量根管工作长度。熟练掌握根管测量仪的应用技术,应用手动或机动的根管预备器械,要取得根管治疗的成功,必须要求每个步骤都精准,包括开髓,根管预备,根管冲洗及封药及根管充填。
Objective To investigate the causes of failure of root canal therapy and clinical measures. Methods The clinical analysis of the causes of failed root canal treatment in 160 cases of first and second molars was performed. Results Postoperative pain swelling in 10 cases, mechanical fracture in 16 cases, medullary cavity perforation and root canal puncture in 16 cases, root canal under filling and superficial root canal filling in 78 cases, 22 cases of root canal omission, root canal in 6 cases, tooth fold 12 cases. Conclusions Root canal therapy should master the technique of root canal preparation, avoid the formation of steps, take radiographs before treatment and after treatment, and if necessary, put enlarged needles to take radiographs and measure the working length of root canal.
