目的探讨血浆同型半胱氨酸(HCY)、脂蛋白(a)[LP(a)]与冠心病(CHD)发生发展的相关性。方法 CHD患者来自苏州市立医院心内科根据国际CHD诊断标准诊断的住院患者,并根据冠状动脉造影术冠脉病变支数进行分为1,2,3支血管病变组;正常对照组来自该院体检中心健康体检人员;研究对象年龄、性别匹配,具有可比性。CHD组根据冠状动脉造影术冠脉病变支数用循环酶法测定血浆HCY水平,用免疫透射比浊法测定血浆LP(a)水平。结果 CHD组HCY、LP(a)水平均值都高于正常对照组,差异有统计学意义(P<0.01)。0,1,2,3支血管病变组HCY、LP(a)浓度呈逐级增高趋势但两者增高程度有一定的差异。结论 HCY、LP(a)与冠脉粥样病变的发生、发展密切相关,是CHD的两个危险因素,如两者联合检测对冠心病的诊断和治疗预后判断具有一定的临床意义。
Objective To investigate the relationship between plasma homocysteine (HCY), lipoprotein (a) [LP (a)] and the development of coronary heart disease (CHD). Methods CHD patients from inpatient department of Suzhou Municipal Hospital were diagnosed according to international CHD diagnostic criteria of inpatients and coronary artery lesions according to the number of coronary artery lesions were divided into 1,2,3 branch vascular lesions group; Center health examination staff; study subjects age, gender match, comparable. Plasma HCY levels were measured by cyclic enzymatic method in CHD group according to the number of coronary lesions in coronary angiography, and plasma LP (a) level was measured by immunoturbidimetry. Results The mean levels of HCY and LP (a) in CHD group were significantly higher than those in normal control group (P <0.01). The concentrations of HCY and LP (a) in 0, 1, 2 and 3 vessel lesions showed a gradual increase trend, but there was a certain difference between the two. Conclusions HCY and LP (a) are closely related to the occurrence and development of coronary atherosclerosis. They are two risk factors of CHD. For example, the combined detection of HCY and LP (a) has some clinical significance in the diagnosis and prognosis of coronary heart disease.
