目的探讨胰头癌行胰十二指肠切除术的疗效及其影响因素。方法对130例胰头癌行胰十二指肠切除术患者的术后并发症、死亡率、术后生存情况及影响因素进行调查。结果手术损伤肠系膜血管及门静脉3例;手术并发症45例(34.6%);手术死亡12例(9.2%)。术后1年、3年和5年存活率分别为44.6%、11.5%和2.3%;肿瘤直径、淋巴结转移和切缘状态是影响患者生存的主要因素,而癌分化程度和患者年龄对5年存活率无明显影响(P>0.05)。结论胰头癌施行胰十二指肠切除术后,并发症及死亡率高,5年存活率低,远近期效果均不十分满意。
Objective To investigate the efficacy and influencing factors of pancreatoduodenectomy for pancreatic head cancer. Methods Postoperative complications, mortality, postoperative survival and influencing factors were investigated in 130 patients with pancreatic head cancer undergoing pancreaticoduodenectomy. Results The surgical injury of mesenteric vessels and portal vein occurred in 3 cases; surgical complications occurred in 45 cases (34.6%); surgical death occurred in 12 cases (9.2%). The 1-year, 3-year, and 5-year survival rates were 44.6%, 11.5%, and 2.3%, respectively; tumor diameter, lymph node metastasis, and margin status were the main factors affecting survival, and the degree of cancer differentiation and patient age were 5 years. The survival rate had no significant effect (P>0.05). Conclusion Pancreatoduodenectomy for pancreatic head cancer has high complications and mortality, 5-year survival rate is low, and the long-term and short-term effects are not very satisfactory.
