目的:分析采用多普勒超声诊断对矽肺患者慢性肺源性心脏病右室病变的预测价值。方法:抽取河南省职业病防治研究院2019年1月至2020年8月收治的矽肺伴慢性肺源性心脏病患者共40例作为A组,另抽取同期收治的不伴慢性肺源性心脏病的矽肺患者共40例作为B组。采用多普勒超声技术分别测定两组患者右室不同节段心肌组织曲线的相关参数,如收缩期峰值应变(PSs)、达-峰值速度时间(TTPV)、收缩期峰值应变率(PSRs)与收缩期峰值位移(PDs),并采用Pearson分析法分析右室射血分数(RVEF)与不同节段心肌组织曲线相关参数的相关性。结果:A组患者右室各节段PSs指标低于B组,右室下壁中段、前壁中段和基底段的PSRs指标低于B组,右室前壁中段和基底段、侧壁基底段、下壁基底段PDs指标低于B组,右室侧壁心尖段TTPV指标低于B组,差异均有统计学意义(n P均<0.05)。A组患者的RVEF值与右室下壁基底段PDs值、各节段PSs值、侧壁中段和前壁基底段PSRs值均呈正向相关(n P均<0.05)。n 结论:采用多普勒超声手段有助于预测矽肺患者慢性肺源性心脏病右室病变的发生,可用于判断患者的右室壁心肌收缩能力。“,”Objective:To analyze the value of Doppler ultrasound in predicting right ventricular lesions in patients with chronic pulmonary heart disease caused by pneumosilicosis.Methods:A total of 40 pneumosilicosis patients with chronic cor pulmonale in Henan Institute for Occupational Health from January 2019 to August 2020 were selected as group A, and 40 pneumosilicosis patients without chronic cor pulmonale in the same period were selected as group B. Doppler ultrasound technology was used to determine the relevant parameters of different segments of right ventricular myocardium in two groups, including peak systolic strain (PSs), time to peak velocity (TTPV), peak systolic strain rate (PSRs), and peak systolic displacement (PDs). Pearson analysis was used to analyze the relationship between the right ventricular ejection fraction (RVEF) and the parameters of curves at different segments.Results:The PSs indexes of right ventricle each segment in group A were lower than those in group B (n P<0.05). The PSRs indexes of middle segment inferior wall, middle segment anterior wall, and basal segment right ventricle in group A were lower than those in group B (n P<0.05). The PDs index of middle segment and basal segment of anterior wall, lateral wall basal segment and inferior wall basal segment in group A were lower than those in group B (n P<0.05). The TTPV index of lateral wall apical segment of right ventricle were lower than those in group B (n P<0.05). RVEF value in group A was positively correlated with PDs value of right inferior wall basal segment, PSs value of each segment, PSRs value of lateral wall middle segment and anterior wall basal segment (n P<0.05).n Conclusions:Doppler ultrasound is helpful to predict the occurrence of right ventricular lesions in chronic cor pulmonale in patients with pneumosilicosis, and it can be used to determine the myocardial contractility of right ventricular wall.
