目的探讨匹多莫德联合抗痨药物治疗小儿肺结核的疗效分析。方法 36例小儿肺结核患儿,随机分为治疗组和对照组,各18例。对照组应用常规抗结核药物治疗,治疗组在常规治疗基础上加用匹多莫德,观察两组治疗后的临床疗效及免疫指标。结果治疗组总有效率为94.45%,高于对照组的66.7%,差异有统计学意义(P<0.05)。治疗组有2例患儿出现不良反应,不良反应发生率为11.1%,对照组有3例患儿出现不良反应,不良反应发生率为16.7%;两组不良反应发生率比较差异无统计学意义(P>0.05)。两组不良反应均不重,予对症处理后均恢复正常。治疗后,治疗组Th1及Th1/Th2指标高于治疗前,Th2指标低于治疗前,且治疗组各项指标优于对照组,差异均有统计学意义(P<0.01);对照组治疗前后Th1、Th2及Th1/Th2指标比较,差异无统计学意义(P>0.05)。结论匹多莫德主要是通过调节患儿的Thl/Th2细胞平衡,提高患儿的细胞免疫功能,使患儿机体免疫能力得到一定的提高,促进病灶吸收和改善结核中毒症状,其疗效好、安全性高、耐受性良好,值得临床推广应用。
Objective To investigate the efficacy of pidotimod in combination with anti-tuberculosis drugs in the treatment of pediatric pulmonary tuberculosis. Methods Thirty-six children with pulmonary tuberculosis were randomly divided into treatment group and control group, with 18 cases in each. The control group was treated with conventional antituberculosis drugs. The treatment group was given pidotimod on the basis of routine treatment, and the clinical curative effect and immune index of the two groups were observed. Results The total effective rate of the treatment group was 94.45%, which was higher than that of the control group (66.7%), the difference was statistically significant (P <0.05). There were two cases of adverse reactions in the treatment group, the incidence of adverse reactions was 11.1%, 3 cases of children in the control group had adverse reactions, the incidence of adverse reactions was 16.7%; the incidence of adverse reactions was no significant difference between the two groups (P> 0.05). Two groups of adverse reactions are not heavy, to symptomatic treatment were back to normal. After treatment, the Th1 and Th1 / Th2 indexes in the treatment group were higher than those before the treatment, while the Th2 indexes in the treatment group were lower than those before treatment, and the indexes in the treatment group were better than those in the control group (P <0.01) Th1, Th2 and Th1 / Th2 indicators, the difference was not statistically significant (P> 0.05). Conclusion Pidotimod mainly by adjusting the balance of Thl / Th2 cells in children to improve children’s cellular immune function, so that children’s immune capacity has been improved to promote the absorption of lesions and improve the symptoms of tuberculosis, and its good effect, High safety, good tolerance, it is worth clinical promotion and application.
