目的探讨食管癌螺旋CT(SCT)征象与癌组织中Cox-2、nm23表达间的关系。方法44例食管癌患者术前均行SCT扫描,并与手术病理对照,术后标本用免疫组织化学方法检测癌组织中Cox-2、nm23表达情况,用SPSS12.0软件进行对照分析。结果(1)SCT对食管癌有无邻近结构受侵和有无淋巴结转移判断准确率分别为88.6%(39/44)和90.9%(40/44),与手术病理对照一致性好(P<0.0001);(2)Cox-2阳性表达率79.5%(35/44),与SCT征象上癌肿长度、邻近结构侵犯、浸润深度、淋巴结转移呈正相关(P<0.05);(3)nm23阳性表达率68.2%(30/44),与SCT征象上浸润深度、淋巴结转移呈负相关(P<0.05)。(4)Cox-2阳性表达和nm23低表达共同促进食管癌的淋巴结转移。结论SCT扫描能较全面地显示食管癌影像学特征,SCT征象与Cox-2、nm23异常表达关系密切。
Objective To investigate the relationship between the expression of Cox-2 and nm23 in esophageal cancer spiral CT (SCT) signs and cancerous tissues. Methods Forty-four patients with esophageal cancer underwent preoperative SCT scan, and compared with the surgical pathology. The expression of Cox-2 and nm23 in cancer tissue was detected by immunohistochemistry after operation. The control group was analyzed by SPSS12.0 software. Results (1) The diagnostic accuracy of SCT on adjacent cancer invasion and lymph node metastasis was 88.6% (39/44) and 90.9% (40/44) respectively, which was consistent with the surgical pathology control (P < 0.0001). (2) The positive expression rate of Cox-2 was 79.5% (35/44), which was positively correlated with the length of SCT, adjacent structure invasion, depth of invasion and lymph node metastasis (P <0.05) The expression rate was 68.2% (30/44), which was negatively correlated with the depth of invasion and lymph node metastasis on SCT sign (P <0.05). (4) The positive expression of Cox-2 and the low expression of nm23 promote the lymph node metastasis of esophageal cancer. Conclusion SCT scan can show the imaging features of esophageal cancer more completely. The signs of SCT are closely related to the abnormal expressions of Cox-2 and nm23.
