流行性乙型脑炎患儿合并惊厥31例(男性19,女性12,年龄3.6±s2.3a),用氯硝西泮0.05-0.1mg/kg,iv缓注1次,再同剂量im,qd×3d。另外29例患儿(男性17,女性12,年龄3.3±1.9a),用地西泮0.3mg/kg,iv缓注,苯巴比妥5-9mg/kg,im,均qd×3d。结果前组显效率87%,后组38%,止惊时间前组2.2±0.5d,后组2.7±0.6d,前组优于后组(P<0.01)。*P<0.01。不良反应缓慢iv或im氯硝西泮,18例有嗜睡、镇静,1例共济失调伴眩晕,但未见心脏、呼吸抑制情况。讨论氯硝西泮是一种高脂溶性易透过血脑屏障的抗惊厥药,静注后数分钟可达血药浓度峰值并迅速进入脑组织,起效迅速,对惊厥频发者许多病例都是边注射边停止惊厥。本品T约为22-38h,作用持续约6-8h,因此本品具有比地西泮作用迅速,同时又兼有苯巴比妥的持续作用时间长的优点。鉴此,我们应用氯硝西泮治疗小儿流行性乙型脑炎的惊厥频发者。效果令人满意。由于氯硝西泮良好的止惊作用,以及其他药物的积极治疗,我们收治的31例流行性乙型脑炎全部治愈而无1例死亡。目前尚未发现类似地西泮iv时发生的呼吸抑制现象。因?
