背景:糖尿病视网膜病变是糖尿病病程中常见的慢性血管性并发症,其发生过程中与细胞凋亡及其离子水平变化的关系尚需验证。目的:观察糖尿病大鼠视网膜组织中细胞凋亡量和钠、钾离子含量的变化。设计:随机对照实验。单位:柳州市人民医院眼科。材料:实验在柳州市人民医院眼科完成。选用成年纯系雄性Wistar大鼠40只,鼠龄20个月,体质量320~350g,由中山大学医学院动物中心提供。方法:将40只Wistar大鼠随机分成对照组和实验组,每组20只。实验组用链脲佐菌素诱发糖尿病。并在糖尿病发病后分别于第4、6、12、16周检测视网膜细胞凋亡和视网膜组织中钠、钾离子含量的变化;对照组单纯用等量柠檬酸盐缓冲液注射。对2组空腹血糖、糖化血红蛋白、钠、钾离子及糖尿病病程进行相关性分析。主要观察指标:①钠、钾离子含量变化。②空腹血糖浓度、糖化血红蛋白水平。③细胞凋亡量变化。④各指标间相关性分析。结果:实验组视网膜细胞溶解液中在发病后4周可测到细胞凋亡,且随糖尿病病程的延长凋亡程度逐渐加重;视网膜组织内钠离子浓度增加、钾离子浓度明显减少(P<0.01或0.05)。实验组视网膜细胞凋亡的程度与空腹血糖浓度、糖化血红蛋白水平、视网膜内钠离子含量及糖尿病病程呈正相关(r分别为7.584,7.844,7.369,6.246,P均<0.01);而与视网膜内钾离子含量呈高度负相关(r=-7.658,P<0.01)结论:糖尿病大鼠视网膜中存在细胞凋亡和钠、钾离子含量的异常,这些变化是糖尿病视网膜病变的病理基础之一。
BACKGROUND: Diabetic retinopathy is a common chronic vascular complication in the course of diabetes mellitus. The relationship between apoptosis and the changes of its ion level in the pathogenesis of diabetic retinopathy needs to be verified. Objective: To observe the changes of apoptosis and sodium and potassium in retinal tissue of diabetic rats. Design: Randomized controlled experiment. Unit: Liuzhou People’s Hospital of Ophthalmology. Materials: The experiment was performed in Department of Ophthalmology, Liuzhou People’s Hospital. Forty male adult pure Wistar rats were used, aged 20 months and weighing 320-350 g, and were provided by Animal Center of Sun Yat-sen Medical College. Methods: Forty Wistar rats were randomly divided into control group and experimental group, with 20 rats in each group. Experimental group with streptozotocin induced diabetes. The apoptosis of retinal cells and the changes of sodium and potassium in retinal tissue were detected at the 4th, 6th, 12th and 16th weeks after onset of diabetes mellitus. The control group was injected with the same amount of citrate buffer solution respectively. The incidence of fasting blood glucose, glycosylated hemoglobin, sodium, potassium and diabetes duration were analyzed. MAIN OUTCOME MEASURES: ① sodium and potassium ion content changes. ② fasting blood glucose levels, glycosylated hemoglobin level. ③ changes in apoptosis. ④ the correlation between the indicators analysis. Results: Apoptosis was detected 4 weeks after the onset of retinopathy in the experimental group, and the degree of apoptosis increased with the duration of diabetes. The sodium concentration in the retina increased and the potassium concentration decreased significantly (P <0.01 Or 0.05). The degree of retinal cell apoptosis in the experimental group was positively correlated with the fasting blood glucose, glycosylated hemoglobin, the intra-retinal sodium content and the duration of diabetes (r = 7.584, 7.844, 7.369, 6.246, P all <0.01) (R = -7.658, P <0.01) .Conclusion: There are abnormalities of apoptosis and sodium and potassium in the retina of diabetic rats. These changes are one of the pathological basis of diabetic retinopathy.
