目的探讨甘精胰岛素在初诊老年2型糖尿病患者治疗中的应用价值。方法选择2012年12月—2013年12月我院收治的初诊老年2型糖尿病患者80例,将其随机分为对照组和观察组,各40例。所有患者治疗前2周给予胰岛素强化治疗,使患者空腹血糖(FBG)和餐后2 h血糖(PBG)达到参考范围。对照组患者给予门冬胰岛素注射液治疗,观察组患者给予甘精胰岛素治疗,疗程为8周。观察两组患者治疗前后FBG和PBG、空腹和餐后2 h C肽水平及治疗期间低血糖发生率。结果治疗前两组患者FBG和PBG、空腹和餐后2 h C肽水平比较,差异无统计学意义(P>0.05);治疗后观察组患者FBG和PBG低于对照组,空腹和餐后2 h C肽水平高于对照组(P<0.01)。治疗期间观察组患者低血糖发生率为5.0%(2/40),低于对照组为25.0%(10/40)(χ2=5.831,P=0.016)。结论甘精胰岛素治疗初诊老年2型糖尿病降糖疗效显著,能够改善胰岛β细胞功能,降低低血糖发生率,值得在临床推广应用。
Objective To investigate the clinical value of glargine in the treatment of newly diagnosed type 2 diabetic elderly patients. Methods Eighty patients with newly diagnosed type 2 diabetes admitted to our hospital from December 2012 to December 2013 were randomly divided into control group and observation group, 40 cases in each group. All patients received intensive insulin therapy two weeks prior to treatment to achieve a reference range for fasting blood glucose (FBG) and 2 h postprandial blood glucose (PBG). Patients in the control group were given insulin aspart as injection, and patients in observation group were given insulin glargine for 8 weeks. The FBG and PBG, fasting and postprandial 2-h C-peptide levels and the incidence of hypoglycemia during the treatment were observed before and after treatment in both groups. Results There was no significant difference in FBG and PBG levels between the two groups before treatment (P> 0.05). FBG and PBG in the observation group were lower than those in the control group after fasting and postprandial 2 h C peptide level was higher than the control group (P <0.01). During the treatment period, the incidence of hypoglycemia in the observation group was 5.0% (2/40), which was lower than that in the control group (25.0%, 10/40) (χ2 = 5.831, P = 0.016). Conclusion Glargine treatment of newly diagnosed type 2 diabetes mellitus in newly diagnosed patients with type 2 diabetes has a significant therapeutic effect on hypoglycemia, which can improve pancreatic β-cell function and reduce the incidence of hypoglycemia, which is worthy of clinical application.
