目的了解福建省厦门市2002-2009年胰腺癌死亡趋势、分布特征及寿命损失情况,为采取胰腺癌防治干预措施提供参考依据。方法对厦门市2002-2009年胰腺癌死亡登记资料进行分析,通过计算粗死亡率、标化死亡率、累积死亡率、潜在寿命损失年(PYLL)和平均减寿年数(AYLL)等指标评价厦门市居民胰腺癌的死亡及寿命损失情况。结果厦门市居民2002-2009年胰腺癌的平均粗死亡率为2.01/10万,平均标化死亡率为1.26/10万,0~74岁累积死亡率为0.21%;其中厦门市居民2002-2009年胰腺癌平均标化死亡率明显低于2006年中国肿瘤登记地区胰腺癌标化死亡率的3.35/10万(U=12.34,P<0.05);城市居民平均标化死亡率为1.56/10万,明显高于农村居民的0.75/10万(U=3.85,P<0.05);男性居民平均标化死亡率为1.47/10万,明显高于女性居民的1.04/10万(U=2.15,P<0.05);厦门市居民2002-2009年胰腺癌死亡年龄中位数为64.5岁,253例胰腺癌死亡病例中68.38%的死亡者为≥60岁居民,26.88%的死亡者为40~59岁居民,<40岁居民仅占4.74%;厦门市2002-2009年胰腺癌PYLL和AYLL分别为2 912.5和11.5人年,其中男性居民分别为1 807.5和12.2人年,分别高于女性居民的1 105.0和10.5人年;城市居民胰腺癌PYLL为2 157.5人年,高于农村居民的755.0人年,城市居民胰腺癌AYLL为11.0人年,低于农村居民的13.5人年。结论厦门市胰腺癌死亡率较低;胰腺癌主要危及中老年人,对男性、城市居民的早死危害程度较高。
Objective To investigate the mortality trends, distribution characteristics and life loss of pancreatic cancer from 2002 to 2009 in Xiamen, Fujian Province, and to provide a reference for the prevention and treatment of pancreatic cancer. Methods The death registration data of pancreatic cancer from 2002 to 2009 in Xiamen were analyzed. The indexes of crude death rate, standardized mortality rate, cumulative mortality rate, PYLL and AYLL were calculated to evaluate Xiamen City residents of pancreatic cancer death and loss of life. Results The average crude death rate of pancreatic cancer in Xiamen was 2.01 / 100,000 in 2002-2009, the average standardized death rate was 1.26 / 100,000 and the cumulative mortality rate was 0.21% in 0-74 years. Among them, Xiamen residents The average standardized mortality of pancreatic cancer in 2007 was significantly lower than that of the normalized rate of pancreatic cancer in China in 2006 (3.35 / 100000) (U = 12.34, P <0.05). The average standardized death rate of urban residents was 1.56 / (U = 3.85, P <0.05). The average standardized death rate of male residents was 1.47 / lakh, which was significantly higher than 1.04 / lakh of female residents (U = 2.15, P <0.05). The median age of death of patients with pancreatic cancer from 2002 to 2009 in Xiamen was 64.5 years. Among the 253 patients with pancreatic cancer, 68.38% of the patients died were ≥60 years old and 26.88% of them were 40-59 years old Residents only accounted for 4.74% of residents <40 years of age; Xiamen PYLL and AYLL of 2002-2009 were 2 912.5 and 11.5 years, respectively, of which male were 1 807.5 and 12.2 years, respectively, higher than female residents 1 105.0 and 10.5 person-years; PYLL of urban residents was 2 157.5 person-years, which was 755.0 years higher than that of rural residents, and the urban resident AYLL of pancreatic cancer was 11.0 person-years, lower than that of rural residents 3.5 people years. Conclusions The mortality rate of pancreatic cancer in Xiamen is relatively low. The main risk of pancreatic cancer is in the middle-aged and elderly people, and the risk of premature death in male and urban residents is higher.
