目的 对 5 0例左束支传导阻滞 (LBBB)合并左心室肥厚 (LVH) (A组 )的心电图诊断 ,与 4 0例单纯完全性LBBB(B组 )对照。结果 A组RⅠ +SⅢ 、SⅢ >RⅡ 、SV2 、SV3、SV3>SV2 、RV6 >RV5、SV1 +RV5、S V3+RV6 、SV1 +SV6 等指标 ,与B组相比P <0 0 1 ,SV3>2 7mV ,敏感性为 89 2 %,准确性为 85 3%,特异性为 87 9%;其次为SV3>SV2 ,敏感性为 5 8 1 %,准确性为 6 1 6 %,特异性为 73 6 %;SV3+RV6 >4 3mV ,敏感性为 6 8 8%,准确性为 6 3 7%,特异性为 74 1 %;RV6 >RV5,敏感性为 5 2 4%,准确性为 6 0 4%,特异性为 70 6 %。结论 SV3、SV3+RV6 >4 3mV、SV3>SV2 、RV6 >RV5、QRS >0 1 5s是LBBB合并LVH的有效参数。
Objective To evaluate the electrocardiographic diagnosis of 50 cases of left bundle branch block (LBBB) with left ventricular hypertrophy (LVH) (group A) and 40 cases of simple complete LBBB (group B). Results The indexes of RⅠ + SⅢ and SⅢ> RⅡ, SV2, SV3, SV3> SV2, RV6> RV5, SV1 + RV5, S V3 + RV6 and SV1 + SV6 in group A were significantly lower than those in group B > 27mV, the sensitivity was 89.2%, the accuracy was 85.3% and the specificity was 87.9%. The second was SV3> SV2 with a sensitivity of 58.1% and an accuracy of 61.6%. The specificity was 73 6%; SV3 + RV6> 43mV, the sensitivity was 68.8%, the accuracy was 6 3 7%, the specificity was 74 1%; RV6> RV5, the sensitivity was 52.4% and the accuracy was 6 0 4%, specificity of 70 6%. Conclusion SV3, SV3 + RV6> 4 3mV, SV3> SV2, RV6> RV5, QRS> 0 15s are effective parameters of LBBB combined with LVH.
