目的观察矽肺患者氧化应激指标的变化,探讨矽肺发生发展的机制。方法对100例对照组、200例接尘组,32例矽尘作业观察对象(原诊断0+患者)组及130例矽肺组分别测定血清中氧化应激指标:超氧化物歧化酶(SOD)、一氧化氮(NO)、谷胱甘肽过氧化物酶(GSH-Px)、总抗氧化能力(T-AOC)、一氧化氮合酶(NOS)及脂质过氧化物丙二醛(MDA)含量。结果与对照组比较,接尘组与矽肺组NO水平、GSH-Px活力均明显升高,SOD水平明显降低(P<0.05,P<0.01);与对照组、接尘组比较,矽肺组T-AOC、NOS、MDA均明显升高(P<0.05,P<0.01);矽肺组较接尘组GSH-Px活力明显升高(P<0.01)。与观察对象组和Ⅰ期矽肺组比较,Ⅲ期矽肺组GSH-Px活力明显升高(P<0.05)。Pearson相关分析显示血清中GSH-Px水平与矽肺分期、分组、接尘工龄、工种呈显著正相关(P<0.01)。结论机体氧化和抗氧化系统的失衡与矽肺的发生发展有关;对氧化应激指标的监测有利于预测矽肺发病和评估预后。
Objective To observe the changes of oxidative stress index in patients with silicosis and to explore the mechanism of silicosis development. Methods Serum levels of oxidative stress, superoxide dismutase (SOD), nitric oxide synthase (SOD) and nitric oxide synthase (SOD) were measured in 100 cases of control group, 200 cases of dust collection group, 32 cases of silicosis observation group (original diagnosis 0+ , Nitric oxide (NO), glutathione peroxidase (GSH-Px), total antioxidant capacity (T-AOC), nitric oxide synthase (NOS) and lipid peroxide malondialdehyde MDA) content. Results Compared with the control group, NO level and GSH-Px activity in the dust-exposure group and the silicosis group were significantly increased and SOD level was significantly decreased (P <0.05, P <0.01). Compared with the control group and the dust- -AOC, NOS and MDA were significantly increased (P <0.05, P <0.01). The activity of GSH-Px in silicosis group was significantly higher than that in control group (P <0.01). Compared with the observation group and stage I silicosis group, the activity of GSH-Px in stage III silicosis group was significantly increased (P <0.05). Pearson correlation analysis showed that GSH-Px levels in serum were significantly and positively correlated with silicosis stage, grouping, the occupational exposure to dust, and work types (P <0.01). Conclusion The imbalance of oxidative and antioxidant systems in the body is related to the occurrence and development of silicosis. The monitoring of oxidative stress is helpful to predict silicosis and assess prognosis.
