目的从微循环角度探讨老龄心脏疾患大鼠对中大型手术的耐受性。方法老龄大鼠注射垂体后叶素复制心肌缺血模型,观察胃大部切除术对老龄心肌缺血大鼠围手术期脊斜肌微循环的变化。结果有心脏疾患的大鼠手术后1h血压明显不降,微循环中第三级动、静脉的血流速度、血液流量和毛细血管开放数明显减少,动、静脉血管口径无明显变化。无心肌缺血的对照老年大鼠手术后,其血压和上述微循环参数无明显改变。结论根据本实验推测老年患者只要器官功能尚正常,可耐受中、大型腹部手术,但有心肌缺血表现时手术会带来机体微循环的严重紊乱,需要加强围手术期的监护和处理。
Objective To investigate the tolerance of middle-scale and large-scale surgery in aged heart disease rats from the perspective of microcirculation. Methods The aged rats were injected with pituitrin into the myocardium to observe the changes of the microcirculation during the perioperative period in the aged myocardial ischemia rats after subtotal gastrectomy. Results The blood pressure at 1 hour after operation in rats with heart disease was significantly decreased. The blood flow velocity, blood flow and the number of open capillaries in the third stage of arterial and venous microcirculation were significantly decreased. There was no significant change in arteriovenous and venous caliber. After the operation of the elderly rats without myocardial ischemia, the blood pressure and the parameters of microcirculation mentioned above had no significant changes. Conclusion According to this experiment, as long as the elderly patients with normal organ function can be tolerated in large abdominal surgery, but the performance of myocardial ischemia when the operation will bring about serious microcirculation of the disorder, the need to strengthen perioperative care and treatment.
