目的:为了寻找青年脑卒中患者的共同危险因素,探讨不明原因青年脑梗死和脑出血中血浆蛋白Z(plasmaproteinZ,PZ)水平的变化及与年龄、体质量指数及纤维蛋白原的关系。方法:采用病例对照研究,共纳入天津渤海石油医院神经内科收治的33例不明原因青年脑梗死患者、39例不明原因青年脑出血患者,同时纳入78例体检者作为对照,以上对象抽取静脉血,采用酶联免疫吸附双抗体夹心法测定其血浆PZ水平。结果:脑梗死组PZ水平为(2.85±0.69)mg/L,显著高于对照组(2.07±0.80)mg/L,差异有非常显著性意义(q=6.1977,P<0.01);脑出血组PZ水平显著低于对照组(P<0.01);脑出血组PZ水平为(1.22±0.53)mg/L,显著低于对照组(q=4.6448,P<0.01)。脑梗死组PZ水平与年龄、体质量指数、纤维蛋白原水平正相关(r分别为0.505,0.436,0.620,P<0.05)。结论:PZ水平的升高/降低可能与青年不明原因脑卒中有关,但进一步的结论还有待研究。
OBJECTIVE: To explore the common risk factors of stroke in young patients and to explore the changes of plasma protein Z (PZ) level in young patients with unexplained cerebral infarction and intracerebral hemorrhage and their relationship with age, body mass index and fibrinogen. Methods: A case-control study was conducted in 33 cases of cerebral infarction of unknown cause admitted to Department of Neurology, Tianjin Bohai Oil Hospital and 39 cases of cerebral hemorrhage of unknown causes in young patients. 78 subjects were included in the study. Venous blood, Plasma PZ levels were measured by sandwich enzyme-linked immunosorbent assay (ELISA). Results: The PZ level in cerebral infarction group was (2.85 ± 0.69) mg / L, which was significantly higher than that in control group (2.07 ± 0.80) mg / L, the difference was significant (q = 6.1977, P <0.01) (P <0.01). PZ level in cerebral hemorrhage group was (1.22 ± 0.53) mg / L, which was significantly lower than that in control group (q = 4.6448, P <0.01). The level of PZ in cerebral infarction group was positively correlated with age, body mass index and fibrinogen level (r = 0.505,0.436,0.620, P <0.05). Conclusion: The increase / decrease of PZ level may be related to stroke in young people with unexplained stroke. However, further research remains to be done.
