目的 探讨孕期氯化消毒副产物内暴露标志血三卤甲烷(trihalomethanes,THMs)浓度与胎儿出生体重之间的关系.方法 对2015年3-6月于孝南区妇幼保健院行产检的312名孕早期孕妇进行问卷调查,分别在孕早、中、晚采集其外周静脉血,并记录胎儿出生体重.采用多元线性回归模型分析孕期全血中四种THMs[三氯甲烷(chloroform,TCM),一溴二氯甲烷(bromodichloromethane,BDCM),二溴一氯甲烷(dibromochloromethane,DBCM)和三溴甲烷(bromoform,TBM)]平均浓度与新生儿出生体重之间的关系.结果 新生儿的平均出生体重为(3 270±393)g.多元线性回归模型结果显示,孕期全血TBM、溴代三卤甲烷(Br-THMs,BDCM、DBCM和TBM的浓度之和)和总三卤甲烷(TTHMs;Br-THMs和TCM浓度之和)浓度升高与新生儿出生体重降低存在建议性剂量-效应关系(趋势性P值<0.10).结论 孕期THMs暴露可能会引起新生儿出生体重降低.“,”Objective To investigate the associations between internal dose of disinfection by-products trihalomethanes (THMs) during gestation and birth weight.Methods During March to June 2015,a total of 312 early pregnant women who presented to the Maternal and Child Care Service Centre of Xiaonan District were recruited to complete a questionnaire and provided repeated blood samples during early,mid-,and late pregnancy.Multivariable linear regression models were used to assess the associations between average levels of 4 THMs [i.e.,chloroform (TCM),bromodichloromethane (BDCM),dibromochloromethane (DBCM) and bromoform (TBM)] and birth weight.Results The mean birth weight of the infants were (3 270± 393) g.The geometric mean of the levels of TCM 、BDCM、DBCM、TBM 、Br-THMs (the sum of BDCM、DBCM and TBM) and TTTHMs (total THMs;the sum of Br-THMs and TCM) in whole blood of participants were 9.02,0.77,0.69,3.52,5.60 and 17.37 ng/L,respectively.Multivariable linear regression analysis showed suggestive dose-effect relationships between elevated blood TBM,Br-THMs,TTHMs and reduced birth weight (P for trend<0.10).The pregnant women in the highest tertiles of TBM,Br-THMs and TTHMs showed significant decreases in the birth weight of 87.60g (95%CI:-175.33,0.12),78.21g (95%CI:-165.84,9.42) and 74.74g (95%C1:-162.76,13.28),respectively,compared with that of the women in the lowest tertiles.Conclusions Exposure to THMs during pregnancy may cause decreased birth weight.
