目的探讨乳腺癌全数字化X线摄影表现与癌细胞Ki-67表达的相关性。方法原发性乳腺癌病例76例,术前均行X线摄影检查,由3位有经验的影像诊断医师阅片,根据相关表现分组。免疫组化采用PV-9000法,测定癌细胞Ki-67表达情况,分析乳腺癌的影像学表现与Ki-67表达的相关性。结果在76例乳腺癌组织中,Ki-67总阳性率为72.2%,乳腺癌影像学表现类型与Ki-67阳性表达无关(P>0.05);Ki-67阳性表达与乳腺癌肿块形态、肿块边缘无关(P>0.05);而与肿块大小有关(分别为χ2=5.26,P<0.05和χ2=6.22,P<0.05);Ki-67阳性表达率在影像学表现为呈簇状、线样及区域状分布的癌灶中均较高(100.0%),而在散在分布的钙化型乳腺癌中未见表达(χ2=24.00,P<0.01),且与钙化颗粒的形态无相关性(P>0.05);乳腺癌患者同侧腋窝淋巴结转移者的Ki-67增殖指数显著高于无淋巴结转移者(χ2=5.76,P<0.05)。结论乳腺癌X线表现与Ki-67表达有一定相关性,可根据乳腺癌X线表现对癌灶的生物学行为和预后进行评估。
Objective To investigate the relationship between full digital radiography and Ki-67 expression in breast cancer. Methods A total of 76 cases of primary breast cancer were examined by X-ray before operation. Three experienced doctors diagnosed the diagnosis of radiologists’ readings, and classified them according to their relative performance. Immunohistochemistry was used to determine the expression of Ki-67 in cancer cells by PV-9000. The correlation between Ki-67 expression and imaging features was analyzed. Results In 76 cases of breast cancer, the positive rate of Ki-67 was 72.2%. The type of breast cancer was not related to the expression of Ki-67 (P> 0.05). The positive expression of Ki-67 was correlated with the morphology of breast cancer, (P> 0.05). The positive expression rate of Ki-67 in the imaging showed clusters, line-like (Χ2 = 24.00, P <0.01), and had no correlation with the morphology of calcified granules (P <0.01), but not with the distribution of calcified granuloma > 0.05). The Ki-67 proliferation index of breast cancer patients with ipsilateral axillary lymph node metastasis was significantly higher than those without lymph node metastasis (χ2 = 5.76, P <0.05). Conclusion The expression of Ki-67 in breast cancer is related to the expression of Ki-67. According to the X-ray findings of breast cancer, the biological behavior and prognosis of the cancer can be evaluated.
