目的通过本次研究旨在确定MMP-9基因多态性与河北承德市满族人胃癌发病易感性的关系。方法应用前瞻性病例-对照研究,采集98例胃癌病人和100例健康人的静脉血,采用蛋白酶K消化-饱和氯化钠盐析法提取外周血白细胞DNA之后,应用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)方法检测MMP-9基因启动子区-1562C/T多态位点的基因型。数据资料分析采用SPSS11.7版软件包,以P﹤0.05为差异有统计学意义。比较对照组与病例组人群各位点基因型频率的观察值,两组病例基因型分布比较采用行×列表卡方检验。不同基因型发病相对风险度用比值比(OR)及其95%可信区间(CI)表示。结果 (1)PCR产物MMP-9基因片段酶切电泳结果显示与PCR产物测序结果相一致。(2)MMP-9-1562C/T多态位点C和T等位基因频率在胃癌患者与健康对照组间差异有统计学意义(P﹤0.05);C/C与(C/T+T/T)基因型频率在患者组与健康对照组间差异有统计学意义(P﹤0.05);C/C基因型与携带T等位基因的基因型(C/T+T/T)相比会降低胃癌的发病风险,OR值为0.532(95%CI=0.298~0.948)。(3)MMP-9基因型频率分布与胃癌TNM分期、淋巴结转移有关,而与肿瘤分化程度和浸润深度无关。结论 (1)河北省承德市满族人MMP-9启动子基因限制性片段长度多态-1562(C→T)与胃癌的易感性有关系。(2)MMP-9基因型与胃癌患者的TNM分期、淋巴结转移有关系,主要影响胃癌的进展和转移。
Objective To determine the relationship between MMP-9 gene polymorphism and susceptibility to Manchurian gastric cancer in Chengde City of Hebei Province through this study. Methods A prospective case-control study was conducted in 98 patients with gastric cancer and 100 healthy controls. The peripheral blood leukocyte DNA was extracted by proteinase K digestion-saturated sodium chloride salting-out method and then analyzed by polymerase chain reaction-restriction The genotypes of -1562C / T polymorphism in the promoter region of MMP-9 gene were detected by PCR-RFLP. Data analysis using SPSS11.7 software package to P <0.05 was considered statistically significant. Comparison of the control group and the case group of the crowd observed the frequency of genotype frequencies, genotype distribution of two groups were compared using row × list chi-square test. The relative risk of different genotypes was expressed as odds ratio (OR) and 95% confidence interval (CI). Results (1) PCR product MMP-9 gene fragment electrophoresis results showed that the PCR product sequencing results are consistent. (2) The frequency of C and T alleles in MMP-9-1562C / T polymorphism sites was significantly different between gastric cancer patients and healthy controls (P <0.05); C / C and (C / T + T (P <0.05). The genotype frequency of C / C genotypes (C / T + T / T) was significantly higher than that of T allele Will reduce the risk of gastric cancer, OR was 0.532 (95% CI = 0.298 ~ 0.948). (3) The frequency distribution of MMP-9 genotype was associated with TNM staging and lymph node metastasis of gastric cancer, but not with tumor differentiation and depth of invasion. Conclusion (1) The restriction fragment length polymorphism -1562 (C → T) of the MMP-9 promoter in Manchu people in Chengde City, Hebei Province is related to the susceptibility to gastric cancer. (2) The genotype of MMP-9 is related to TNM staging and lymph node metastasis in gastric cancer patients, mainly affecting the progress and metastasis of gastric cancer.
