目的研究多层螺旋CT肺容积指标与肺功能(PFT)的相关性及其在阻塞性肺通气功能障碍中的特点。方法正常对照组38例,阻塞性肺通气功能障碍24例,其中20例诊断慢性阻塞性肺疾病(COPD),均在1周内完成PFT及CT检查。采用多层螺旋CT(MSCT)于最大吸气末和最大呼气末分别扫描全肺,用Pulmo肺定量软件测定全肺容积。结果MSCT的肺容积指标其中的最大吸气末容积(Vin)与PFT的肺总量的相关性最佳(r=0.961,P<0.01);最大呼气末容积(Vex)与残气量(RV)、容积差(Vin-Vex)与最大肺活量(MVC)、容积比(Vex/Vin)与残气量与肺总量比(RV/TLC)也存在着较好的相关性(r=0.899,P<0.01;r=0.772,P<0.01;r=0.614,P<0.01)。阻塞性肺通气功能障碍者Vin、Vex、Vex/Vin均有显著升高(P<0.058),与PFT的表现相一致。结论MSCT测定的肺容积指标与PFT有很好的相关性,可以用于评估COPD患者的肺功能状况。
Objective To study the correlation between pulmonary volume index and pulmonary function (PFT) in multi-slice spiral CT and its characteristics in obstructive pulmonary ventilation dysfunction. Methods Thirty-eight patients with normal control group, 24 patients with obstructive pulmonary dysfunction, including 20 patients diagnosed as chronic obstructive pulmonary disease (COPD), completed PFT and CT within one week. Whole lungs were scanned by MSCT at the end of maximal inspiration and maximal end expiratory, respectively. Pulmonary volume was measured by Pulmo lung quantitative software. Results The correlation between the maximum end-inspiratory volume (Vin) and the total lung volume of PFT was the best (r = 0.961, P <0.01). The maximum end-expiratory volume (Vex) ), There was also a good correlation between RV-TLC, RV-TLC, Vein / Veg and MVC, RV / <0.01; r = 0.772, P <0.01; r = 0.614, P <0.01). Vin, Vex, Vex / Vin of obstructive pulmonary dysfunction were significantly increased (P <0.058), consistent with the performance of PFT. Conclusions The lung volume index measured by MSCT has a good correlation with PFT and can be used to evaluate the pulmonary function status in COPD patients.
