目的：比较单、双侧钢板内固定术治疗复杂胫骨平台骨折的疗效。方法复杂胫骨平台骨折患者58例，分为双侧钢板内固定术组（双侧组，30例）和单侧钢板内固定术组（单侧组，28例）。比较两组手术时间、术中出血量、住院时间、负重时间、骨折愈合时间及并发症发生率，术后6个月行膝关节活动度（ROM ）及膝关节功能（HSS）评估。结果两组术中出血量相仿（P＞0．05）；双侧组手术时间长于单侧组（P＜0．05），但双侧组住院时间短，骨折愈合和负重早（P＜0．05）。双侧组术后并发症的发生率低于单侧组（13.3％ vs ．35.7％）（P＜0．05）。术后6个月，双侧组膝关节ROM 和 HSS评分高于单侧组[（94.5±16.1）度vs ．（81.5±14.6）度和（74.4±16.5）分vs ．（61.5±13.8）分]（P＜0．05）。结论双侧钢板内固定术治疗复杂胫骨平台骨折疗效确切，并发症少，膝关节功能恢复较好。“,”Objective To compare the efficacy of internal fixation with single-and double-sided plate in treating complex tibial plateau fractures .Methods Fifty-eight patients with complex tibial plateau fractures were treated by internal fixation with double-sided plate (group D ,30 cases ) or single-sided plate(group S ,28 cases) .The operation time ,operative blood loss ,hospital stay ,loading time ,fracture healing time ,and incidence of complications were compared between two groups .The range of motion of the knee joint and knee functions HSS score were evaluated in 6 months after surgery .Results The operative blood loss of two groups was similar (P>0 .05) .The operation time of group D was longer than that of group S(P<0 .05) .Compared to group S ,the patients in group D had shorter hospital stay ,earlier fracture healing and loading time ( P<0 .05 ) .The incidence of complications was lower in group D than that in group S (13.3% vs .35.7% )(P<0 .05) .Compared to group S ,the patients in group D had higher range of motion of the knee joint [(94.5 ± 16.1) degrees vs .(81.5 ± 14.6) degrees](P<0 .05) and knee functions HSS score[(74.4 ± 16.5) points vs .(61.5 ± 13.8) points](P<0 .05) .Conclusion Internal fixation with double-sided plate is effective for the treatment of complex tibial plateau fracture with less complications and better recovery of knee function .
