目的探讨B-Lynch缝合术治疗剖宫产产后出血的临床疗效及术后出血量的变化。方法回顾分析2014年9月-2016年8月医院收治的剖宫产产后出血患者96例,按照患者产后出血处理方式将其分为观察组和对照组,每组各48例,观察组患者实施B-Lynch缝合术治疗,对照组患者按照常规血窦缝合法加用宫腔纱布填塞止血;对比两组患者的临床疗效,并采取容积法评估患者术后出血量改善情况。结果观察组患者在行产后止血治疗24 h内出血量、止血操作耗时、恶露干净时间以及月经来潮时间均明显低于对照组,差异有统计学意义(P<0.01);术后Hb指标明显高于对照组,术后恢复期输血例数与感染、产褥等并发症率均明显低于对照组,差异均有统计学意义(P<0.01)。结论 B-Lynch缝合术治疗剖宫产产后出血疗效确切,可在短时间内控制产妇出血量,改善宫腔肌层收缩力,促进止血效果提升,降低术后并发症风险,保证患者预后恢复质量,值得推广应用。
Objective To investigate the clinical efficacy and postoperative bleeding of B-Lynch suture in treating cesarean section postpartum hemorrhage. Methods Ninety-six patients with cesarean section postpartum hemorrhage admitted from September 2014 to August 2016 in our hospital were retrospectively analyzed. According to the way of postpartum hemorrhage, the patients were divided into observation group and control group, with 48 cases in each group and the patients in observation group B-Lynch suture, the control group patients in accordance with conventional sinusoidal suture plus uterine gauze packing hemostasis; compared the clinical efficacy of two groups of patients, and take volume method to assess the patient's postoperative bleeding to improve the situation. Results In the observation group, the amount of hemorrhage, the time of hemostasis, the clean-up time of lochia and the period of menstrual cramps were significantly lower than those of the control group within 24 h after hemostasis, the difference was statistically significant (P <0.01); the postoperative Hb index was significantly higher In the control group, the number of blood transfusion and infection, puerperal and other complications were significantly lower in the control group, postoperative recovery period, the difference was statistically significant (P <0.01). Conclusion B-Lynch suture is effective in treating cesarean section postpartum hemorrhage, which can control the amount of maternal bleeding in a short period of time, improve contractility of the uterine myometrium, promote the hemostatic effect, reduce the risk of postoperative complications and ensure the quality of prognosis recovery , It is worth promoting application.
