目的:观察丹参注射液联合神经节苷脂治疗新生儿缺氧缺血性脑病的疗效及安全性。方法:将72例缺氧缺血性脑病患儿随机分为对照组35例和治疗组37例,2组患儿均给予吸氧、维持正常的通气及换气功能、控制惊厥、降颅压、维持血糖及酸碱平衡等对症支持治疗。对照组在此基础上使用胞二磷胆碱注射液、脑蛋白水解物注射液治疗,治疗组在对照组治疗基础上加用丹参注射液、单唾液酸四己糖神经节苷脂钠注射液治疗。2组患儿均以治疗10天为1疗程,观察1疗程。治疗前后评定新生儿神经行为(NBNA)评分,检测血清白细胞介素(IL)-18、IL-1β和肿瘤坏死因子(TNF)-α水平。结果:治疗组总有效率为94.59%,高于对照组的总有效率(82.86%),差异有显著性意义(P<0.05)。治疗10天后,2组患儿NBNA评分均较治疗前升高(P<0.05,P<0.01),治疗组改善更显著;治疗组NBNA评分高于对照组(P<0.05)。血清IL-18、IL-1β和TNF-α水平均较治疗前降低(P<0.05,P<0.01),治疗组降低更显著;2组3项指标分别比较,差异均有显著性意义(P<0.05)。结论:丹参注射液联合神经节苷脂治疗新生儿缺氧缺血性脑病,能显著改善患儿的神经功能,降低炎性因子的水平,疗效确切,安全可靠,值得推广。
Objective: To observe the efficacy and safety of Danshen injection combined with ganglioside in the treatment of neonatal hypoxic-ischemic encephalopathy. Methods: Seventy-two children with hypoxic-ischemic encephalopathy were randomly divided into control group (n = 35) and treatment group (n = 37). Both groups were given oxygen, maintain normal ventilation and ventilation, control seizures, reduce intracranial pressure , Maintain blood sugar and acid-base balance symptomatic and supportive treatment. On the basis of this, the control group was treated with citicoline injection and brain protein hydrolyzate injection. The treatment group was treated with Salvia miltiorrhiza injection and monosialotetrahexosyl ganglioside sodium injection treatment. Two groups of children were treated for 10 days as a course of treatment, observed a course of treatment. Neonatal neurobehavioral (NBNA) scores were assessed before and after treatment, and serum levels of interleukin (IL) -18, IL-1β and tumor necrosis factor (TNF) -α were measured. Results: The total effective rate of the treatment group was 94.59%, higher than that of the control group (82.86%), the difference was significant (P <0.05). After 10 days of treatment, the NBNA scores of two groups were significantly higher than those before treatment (P <0.05, P <0.01), and the treatment group improved significantly. The NBNA score of the treatment group was higher than that of the control group (P <0.05). The levels of serum IL-18, IL-1β and TNF-α in the two groups were significantly lower than those before treatment (P <0.05, P <0.01) <0.05). Conclusion: Salvia miltiorrhiza injection and ganglioside treatment of neonatal hypoxic-ischemic encephalopathy can significantly improve children’s neurological function, reduce the level of inflammatory cytokines, the effect is accurate, safe and reliable, it is worth promoting.
