目的了解河北省循环的麻疹野病毒抗原变异情况以评估疫苗接种成效;掌握麻疹疫苗的保护效果,为制定消除麻疹的策略和措施提供科学依据。方法采集40份疑似麻疹患者咽拭子标本进行麻疹病毒分离,通过荧光定量PCR鉴定麻疹病毒;用健康人血清和麻疹患者血清分别中和麻疹疫苗株和野毒株,分别计算中和抗体滴度,比较毒株在中和抗体滴度上是否存在差异。结果用40份麻疹患者血清,分别中和麻疹野毒株(Hebei11-03和Hebei11-04)和疫苗株(S191),中和抗体GMT值分别为1∶59.9、1∶59.7、1∶60.4,差异无统计学意义(P>0.05);用40份健康人血清同步进行中和实验,抗体GMT值分别为1∶56.0、1∶56.4、1∶69.3,抗体滴度差异有统计学意义(P<0.05)。结论目前使用的麻疹疫苗产生的免疫力对于流行的麻疹野毒株仍具有抵抗力。但使用麻疹疫苗产生的麻疹免疫力明显低于野毒株感染所产生的免疫力。
Objective To understand the variation of circulating wild measles virus antigen in Hebei Province to assess the effectiveness of vaccination. To master the protective effect of measles vaccine and provide a scientific basis for the development of strategies and measures to eliminate measles. Methods Thirty swabs of measles virus were collected and measles virus was isolated from 40 patients with measles measles virus. The measles virus was identified by real-time fluorescence quantitative PCR. Measles vaccine and wild strain were respectively neutralized by serum of healthy people and measles patients. The titer of neutralizing antibody , To compare the strains in neutralizing antibody titer whether there is a difference. Results 40 cases of measles patient serum were used to neutralize measles virus strains (Hebei11-03 and Hebei11-04) and vaccine strains (S191) respectively. The neutralizing antibody GMT values were 1:59.9, 1:59.7, 1:60.4, The difference was not statistically significant (P> 0.05). The serum neutralization test was carried out in 40 healthy volunteers. The antibody GMT values were 1:56.0,1:56.4,1:69.3, respectively. The antibody titer was significantly different (P <0.05). Conclusion The immunity generated by the measles vaccine currently used is still resistant to the wild measles virus. However, the measles immunity produced by the measles vaccine was significantly lower than that produced by the wild-type infection.
