目的 :探讨彩色多普勒超声诊断血管性阳萎的应用价值。方法 :应用彩色多普勒超声检查 ,对临床疑阳萎 39例患者的阴茎海绵体动脉血流进行检测 ,观测阴茎海绵体内注射罂粟碱加立其丁之前、后的动脉管径值及注药后 15 min内的收缩期峰值流速 ( Vmax)、舒张末期最小流速 ( Vmin)、阻力指数 ( RI)、搏动指数 ( PI)。结果 :对罂粟碱加立其丁反应良好 7例 ,反应低下 32例 ,反应良好者 Vmax为 ( 41.33± 9.76 ) cm/ s,Vm in为 ( 3.5 2± 1.2 2 ) cm/ s,RI为 0 .94± 0 .0 4,PI为 4.87± 0 .6 0 ;反应低下者中 18例为静脉机能不全 ,10例为动脉供血不足 ,4例为动静脉混合性阳萎 ,其 Vmax分别为 ( 36 .14± 8.2 ) cm / s、 ( 2 2 .2 3± 4.6 ) cm/ s、 ( 2 3.5 7± 5 .0 ) cm/ s,Vmin分别为 ( 7.2 0± 2 .10 ) cm/ s、 ( 2 .5 1± 1.31) cm/ s、 ( 6 .6 8± 1.40 ) cm/ s,RI分别为 0 .75± 0 .0 4、 0 .89± 0 .0 6、 0 .6 6± 0 .0 6 ,PI分别为 2 .0 8± 0 .5 6、 4.5 2± 0 .5 1、 2 .15± 0 .5 5。反应良好与反应低下的静脉机能不全者注药前后阴茎海绵体动脉管径值的增加率大于 2倍 ,而动脉供血不足与动静脉混合性者注药前后阴茎海绵体动脉管径值的增加率小于 6 0 %。结论 :彩色多普勒超声配合罂粟碱加立其丁试?
Objective: To explore the value of color Doppler ultrasound in the diagnosis of vascular impotence. Methods: The color Doppler echocardiography was used to detect the penile cavernous blood flow in 39 patients with suspected clinical signs of phlebitis. Reactive and low response of venous insufficiency before and after injection of penile cavernous arterial diameter increased more than 2 times, while arterial insufficiency and arteriovenous mixed injection of penis before and after the diameter of arterial diameter increases Less than 60%.
