目的 探讨彩色多谱勒超声对乳腺肿物病灶内新生血管阻力指数和搏动指数的测定在肿物鉴别诊断中的价值。方法 观察 46例乳腺恶性肿瘤患者 ( 47个病灶 )、6 9例纤维腺瘤和 7例炎症包块患者及 2 2例乳腺增生患者。病灶的二维图像特征 ,记录其彩色血流的检出率及多普勒定量分析数据 ,对良恶性肿瘤进行比较。结果 良恶性肿物彩色血流检出率分别为 5 9.4%和 89.0 % ,两者差异有统计学意义。( P<0 .0 0 1 ) ,肿物内血管的阻力指数和搏动指数良恶性之间差异有显著意义。以阻力指数≥0 .70和搏动指数≥ 1 .30作为恶性肿瘤的特征 ,具有较理想的鉴别诊断价值 ,其总准确率可达 95 .5 %。结论 彩色血流多普勒分析较仅用二维图像可显著提高对乳腺肿物鉴别诊断的敏感性和特异性。
Objective To explore the value of color doppler ultrasound in the determination of the resistance index and pulsatility index of neovascularization in breast lesions in differential diagnosis of tumors. Methods 46 cases of breast cancer (47 lesions), 69 fibroadenoma and 7 cases of inflammatory mass and 22 cases of breast hyperplasia were observed. The two-dimensional image features of lesions were recorded and the color blood flow detection rate and Doppler quantitative analysis data were recorded to compare benign and malignant tumors. Results The color blood flow detection rate of benign and malignant tumors were 5 9.4% and 89.0 %, respectively. The difference was statistically significant. (P <0. 0 0 1 ), the difference between the resistance index of the blood vessel within the tumor and the beating index was significant. With resistance index ≥ 0.70 and pulsatility index ≥ 1.30 as the characteristics of malignant tumors, it has an ideal value of differential diagnosis, and its total accuracy rate can reach 95.5%. Conclusion Color Doppler flow analysis can significantly improve the sensitivity and specificity of differential diagnosis of breast masses compared to using only two-dimensional images.
