目的研究观察妊娠合并肾衰竭血液透析患者血清单个核白细胞TNF-a、血管紧张素Ⅱ(AngⅡ)表达,讨论妊娠合并肾衰竭患者的微炎症状态与心血管并发症的相关性。方法采用健康组与妊娠合并急性肾衰竭对比,透析患者组选择妊娠合并急性肾衰竭患者,排除急性感染和慢性传染病患者,每周透析4次进行血清TNF-a、AngⅡ表达研究。结果妊娠合并肾衰竭患者进入血液透析治疗随着血液透析时间的延长,一些慢性并发症成为影响患者生存和生命质量的重要原因。外周血单个核细胞受到刺激被活化后,能促使动脉粥样硬化加速形成。并能早期出现单核巨噬细胞系统被激活导致炎性反应,而炎症状态与动脉粥样硬化和心血管疾病密切相关。结论妊娠合并肾衰竭血液透析患者血清单个核白细胞胞分泌的TNF-a、AngII明显升高,妊娠合并肾衰竭患者的微炎症状态与心血管并发症相关。
Objective To investigate the expression of TNF-α and angiotensin Ⅱ (Ang Ⅱ) in serum mononuclear leukocytes in patients with renal failure and hemodialysis during pregnancy. The relationship between microinflammatory state and cardiovascular complications in patients with pregnancy-induced renal failure was investigated. Methods The patients in acute dialysis group were divided into healthy group and acute renal failure group. The patients in acute dialysis group were divided into acute renal failure group and acute infection group. The serum levels of TNF-a and AngⅡ were determined by dialysis four times a week. Results Patients with CRF During Hemodialysis Treatment With the extension of hemodialysis time, some chronic complications have become the important reasons that affect the survival and quality of life of patients. Peripheral blood mononuclear cells are stimulated to be activated, can promote the formation of accelerated atherosclerosis. And early monocyte-derived macrophage system is activated to cause inflammatory reaction, and inflammatory status is closely related to atherosclerosis and cardiovascular diseases. Conclusions The levels of TNF-α and AngII secreted by mononuclear leukocytes in patients with hemodialysis during pregnancy complicated with renal failure are significantly increased. The micro-inflammatory status in patients with pregnancy-induced renal failure is associated with cardiovascular complications.
