目的:探讨影响高血压脑出血血肿清除术后血压变化的因素。方法:对65例高血压脑出血血肿清除术的临床资料采用多因素逐步回归法进行统计学处理。结果:与术后收缩压有关的自变量为:年龄、出血破入脑室、小脑出血、术前收缩压等,且为正相关。与术后舒张压有关的自变量为:小脑出血、出血破入脑室、基底节内侧型、术前舒张压,为正相关。结论:下列情况易出现高血压脑出血血肿清除术后的血压波动:(1)年龄<55岁;(2)术前血压较高的;(3)出血破入脑室;(4)小脑出血;(5)基底节内侧型出血者。
Objective: To explore the factors influencing the changes of blood pressure after hematoma removal in hypertensive intracerebral hemorrhage. Methods: The clinical data of 65 patients with hypertensive intracerebral hemorrhage hematoma were analyzed by multi-factor stepwise regression. Results: The independent variables related to postoperative systolic blood pressure were age, bleeding into ventricle, cerebellar hemorrhage, preoperative systolic pressure and so on. Independent variables related to postoperative diastolic pressure were cerebellar hemorrhage, hemorrhages breaking into the ventricles, medial basal ganglia and preoperative diastolic pressure, which were positively correlated. CONCLUSIONS: Fluctuations in blood pressure after hypertensive intracerebral hemorrhage hematoma are prone to occur in the following cases: (1) age <55 years; (2) preoperative high blood pressure; (3) bleeding into the ventricles; (4) cerebellar hemorrhage; (5) medial basal gangoid bleeding.
