桂萘甲胺(naftifine)是一种新含成的抗真菌药,为丙烯胺的衍化物,在结构上与其它广谱的抗真菌剂无关。它通过抑制角鲨烯环氧化酶,使细胞内产生的角鲨烯蓄积,进而妨碍麦角笛醇的生物合成。与咪唑类药物相比,它作用于麦角笛醇合成的更早阶段。试管内研究发现桂萘甲胺对酵母菌有中度抑菌作用,对念珠菌属最小抑菌浓度为1.5~100μg/ml。本文用双盲临床试验评价了外用桂萘甲胺治疗皮肤念珠菌病的疗效及安全性。经直接镜检及培养确诊的60例皮肤念珠菌病患者,被随机分为2组,1组外用1%桂萘甲胺霜,另1组用赋形剂,每日2次,共用3周。治疗期间及治疗结束后2周内,每1周随访1次。根据红斑、水疮、皲裂、脓疱、丘疹、浸渍、脱屑、结痂、瘙痒、疼痛及烧灼感,评为无、轻度、中度、重度4级,并作真菌镜检及培养。
Naftifine is a new antifungal agent that is a derivative of acrylamides that is structurally independent of other broad spectrum antifungal agents. It inhibits squalene cyclooxygenase and causes the accumulation of squalene in the cells, which hinders the biosynthesis of ergot alpacas. It acts on earlier stages of the synthesis of ergot alcohols than imidazoles. In vitro study found that cinnamic acid naphthalene methylamine moderate inhibitory effect on yeast, the minimum inhibitory concentration of candida was 1.5 ~ 100μg / ml. This article evaluated the efficacy and safety of topical cinnamylmethamine in the treatment of cutaneous candidiasis by double-blind clinical trials. Sixty cases of cutaneous candidiasis diagnosed by direct microscopic examination and culture were randomly divided into two groups. One group received external application of 1% cinnaron methylamine cream and the other group received vehicle twice daily for 3 weeks . During the treatment and 2 weeks after the end of treatment, every 1 week follow-up 1 time. According to erythema, water sores, chapped, pustular, papules, dipping, scaling, crusting, itching, pain and burning sensation, was rated as mild, moderate, severe grade 4, and fungi microscopy and culture.
