目的:分析经颅多普勒超声黑质回声特征对不同分型帕金森病的鉴别诊断价值。方法:抽取2018年3月至2020年4月在郑州颐和医院接受治疗的60例帕金森病患者作为观察组;并选取同期受治疗的58例原发性震颤患者作为对照组。全部患者均接受经颅多普勒超声检查,记录并比较两组双侧中脑总面积、双侧脑黑质(SN)高回声总面积及SN/中脑总面积(S/M);依据帕金森患者不同分型将其分为震颤型、强直型及混合型,比较各组SN高回声总面积及S/M,绘制受试者工作特征曲线(ROC曲线),检验SN回声特征及S/M分别对不同帕金森病分型的诊断价值。结果:观察组与对照组双侧中脑总面积比较差异未见统计学意义(n P>0.05);观察组双侧SN高回声总面积、S/M大于对照组,差异有统计学意义(n P0.05); the SN total hyperechoic area and S/M of the observation group were greater than those of the control group (n P<0.05); among the 60 patients with Parkinson’s disease, there were 25 cases of tremor type, 23 cases of tonic type, and 12 cases of mixed type; the mixed group had the highest SN echo area and S/M, followed by the tonic type and tremor type (n P0.80, the predictive value was ideal; the tremor type and tonic type SN high echo intensity classification were concentrated in grades Ⅰ-Ⅲ, while the mixed type was mainly concentrated in grades Ⅳ-Ⅴ; in addition, mixed-type Parkinson’s disease had the highest SN high echo positive rate, which was higher than that of tremor type and tonic type (n P0.05).n Conclusions:The substantia nigra echo characteristics of different types of Parkinson’s disease are different, clinically, different subtypes of Parkinson’s disease can be identified by observing the characteristics of transcranial Doppler ultrasonography substantia nigra echoes, in order to improve the accuracy of differential diagnosis of different types of Parkinson’s disease.
