目的比较西罗莫司洗脱支架(Cypher 或 Cypher select)和紫杉醇洗脱支架(TAXUS)治疗支架内再狭窄的临床近期及10个月疗效。方法自2002年12月至2005年3月,对253例支架内再狭窄的患者采用了药物洗脱支架(DES)治疗并完成了10个月的临床随访和冠状动脉造影复查。253例中男性218例,女性35例,年龄30~80岁,平均年龄57.2岁。结果 253例(262处病变)中152例使用 Cypher 支架176个,101例使用 TAXUS 支架132个。使用的 Cypher 和 TAXUS 支架的平均直径分别为(2.96±0.27)mm 和(3.05±0.35)mm,P=0.04,平均长度分别为(23.3l±6.68)mm和(23.56±6.54)mm,P=0.745。支架内再狭窄表现为100%闭塞29处,≥90%狭窄143处,<90%狭窄90处。病变类型为 A、B1、B2和 C 型各为9处、45处、73处和135处。PCI 的成功率两组均为100%,住院期间无死亡,Cypher 组主要心脏不良事件(MACE)发生率为2.63%,TAXUS 组为2.97%,P=0.872。10个月临床造影显示在 Cypher 支架和 TAXUS 支架组中造影再狭窄率分别为14.0%和29.4%,P=0.075,MACE 发生率分别为6.7%和16.0%,P=0.031。结论应用 Cypher 和 TAXUS 支架治疗支架内再狭窄有良好的近期临床疗效,10个月疗效 Cypher 支架优于 TAXUS 支架。
Objective To compare the short-term and long-term efficacy of sirolimus-eluting stents (Cypher or Cypher select) and paclitaxel-eluting stents (TAXUS) in the treatment of in-stent restenosis. Methods From December 2002 to March 2005, 253 cases of patients within stent restenosis using drug-eluting stents (DES) and completed treatment of clinical follow-up of 10 months and coronary angiography. 253 cases of male 218 cases, 35 females, aged 30 to 80 years, mean age 57.2 years. Results Among the 253 cases (262 lesions), there were 176 Cypher stents in 152 cases and 132 in TAXUS stents in 101 cases. TAXUS Cypher stent and the average diameter were used (2.96 ± 0.27) mm and (3.05 ± 0.35) mm, P = 0.04, respectively, the average length (23.3l ± 6.68) mm and (23.56 ± 6.54) mm, P = 0.745. In-stent restenosis showed 100% occlusion at 29, ≥90% stenosis at 143, and <90% stenosis at 90. The lesions were type A, type B1, type B2 and type C for each of 9, 45, 73 and 135. PCI success rate was 100% in both groups, no death during hospitalization, Cypher group of major adverse cardiac events (MACE) rate was 2.63%, TAXUS group was 2.97%, P = 0.872.10 month clinical angiography showed the Cypher stent And TAXUS stent group were 14.0% and 29.4%, P = 0.075, MACE incidence was 6.7% and 16.0%, respectively, P = 0.031. Conclusion The application of Cypher and TAXUS stent in the treatment of in-stent restenosis has a good short-term clinical curative effect. The Cypher stent is superior to TAXUS stent in 10 months.
