目的观察双源CT双能量肺灌注成像(dual energy perfusion imaging,DEPI)在疑似肺栓塞(pulmonary embolism,PE)诊断中的应用效果。方法选择2013年4月——2014年9月在黑龙江省医院CT室进行检查的疑似PE患者31例作为研究对象,所有患者均具有肺动脉血管造影术(CT angiography,CTPA)肺动脉诊断图像资料,比较CTPA和DEPI在PE诊断中的成像特点及差异。结果 DEPI在肺叶动脉共反映出25例(89.3%)灌注异常;在肺段水平共反映出60例(98.4%)灌注异常;在肺亚段动脉共反映出51例(92.7%)灌注异常;DEPI总体诊断符合度为94.4%(136/144)。结论 DEPI在不增加对比剂使用剂量、扫描时间和辐射剂量的同时,能够获得肺内血管的形态和功能信息和反映肺内血流的灌注状态信息,能为临床诊断和治疗PE提供更多有价值的信息。
Objective To observe the application of dual-source CT dual-energy perfusion imaging (DEPI) in the diagnosis of suspected pulmonary embolism (PE). Methods Totally 31 patients with suspected PE who were examined in CT room of Heilongjiang Provincial Hospital from April 2013 to September 2014 were selected as study subjects. All patients had pulmonary artery imaging data of CT angiography (CTPA) Imaging Features and Differences of CTPA and DEPI in PE Diagnosis. Results DEPI showed a total of 25 cases (89.3%) of perfusion abnormalities in pulmonary arteries, 60 cases (98.4%) of perfusion abnormalities in lung segments and 51 cases (92.7%) of perfusion abnormalities in pulmonary subarachnoid arteries. The overall diagnosis of DEPI was 94.4% (136/144). Conclusion DEPI can not only increase the dose of contrast agent, scan time and radiation dose, but also can obtain the morphological and functional information of intrapulmonary vessels and the perfusion state information reflecting the intrapulmonary blood flow, which can provide more information for clinical diagnosis and treatment of PE Value information.
