目的:通过对过敏性紫癜(HSP)与紫癜性肾炎(HSPN)患儿瘀血积分、凝血相关指标的观察,探讨瘀血对HSP患儿肾脏损伤的影响。方法:85例患儿按有无尿检异常分为HSP组45例,HSPN组40例。根据中医瘀血症候积分表进行瘀血评分。采用免疫比浊法测D-二聚体(D-Dimer),ELISA法测血浆与尿液纤溶酶原激活物抑制剂-1(PAI-1)。结果:与HSP组比较,HSPN组瘀血积分、尿PAI-1/Cr明显增高(P<0.05)。血D-Dimer、血PAI-1在HSP和HSPN组差异无统计学意义。尿PAI-1/Cr与瘀血积分、24h尿蛋白定量均呈正相关(P<0.05)。结论:瘀血参与了HSP患儿肾脏损伤的发生,尿液中PAI-1水平的增高与HSP肾脏损伤有关。
Objective: To observe the effect of blood stasis on kidney damage in children with HSP by observing the blood stasis score and related indicators of coagulation in children with Henoch-Schonlein purpura (HSP) and purpuric nephritis (HSPN). Methods: Eighty-five children were divided into HSP group (45 cases) and HSPN group (40 cases) according to the presence or absence of urine test. According to TCM ecchymosis score table for stasis score. D-dimer was measured by immunoturbidimetry and plasminogen activator inhibitor-1 (PAI-1) by ELISA. Results: Compared with HSP group, the blood stasis score and urine PAI-1 / Cr in HSPN group were significantly increased (P <0.05). Blood D-Dimer, blood PAI-1 in HSP and HSPN group no significant difference. Urine PAI-1 / Cr had a positive correlation with blood stasis score and 24h urine protein (P <0.05). Conclusion: Stasis is involved in the development of renal injury in children with HSP. The increase of PAI-1 in urine is related to the injury of kidney in HSP.
