患者男性,49岁,右利手,于入院前一天晚出现说话口齿不清,次日晨起穿衣时左手系钮扣笨拙、取物不准。但不伴有头痛、眩晕、肢体力弱或麻木等症状。既往有高血压病史十二年。八年前曾有“一过性失语”。查体:血压130/100毫米汞柱,神清,构音障碍,左侧鼻唇沟略浅,四肢肌力、肌张力正常,步态正常,左手快速轮替动作笨拙,左侧膝反射及踝反射(+++),左侧Chaddock’s征(+),脑膜刺激症(-)。腰穿脑脊液初压170毫米水柱,
