为在气管重建术中应用胸锁乳突肌肌骨膜瓣进行了动物实验。切除犬的1/2周径2~6气管环前壁气管及粘膜组织,用带蒂胸锁乳突肌肌骨膜瓣修复,分别于术后2、4、6月处死动物,观察气管腔修复情况,并取材用光镜、扫描电镜观察。结果表明,用胸锁乳突肌肌骨膜瓣修复气管壁具有良好的支撑力,修复的呼吸道管腔通畅,管壁无瘢痕及肉芽组织形成,移植骨膜表面覆有完整的纤毛柱状上皮,深层有新骨组织形成。临床治疗8例患者,其中瘢痕性气管狭窄5例,甲状腺癌侵犯气管壁2例,气管乳头状瘤恶变1例,均拔除气管套管,恢复正常呼吸功能,随访疗效稳定。提示胸锁乳突肌肌骨膜瓣是理想的喉气管重建材料。
Animal experiments were performed for the application of sternocleidomastoid muscle flap in tracheal reconstruction. Tracheal and mucosal tissues of the anterior wall of tracheal ring of 1/2 ~ 6th tracheal canals were excised from dogs, and the pedicled sternocleidomastoid muscle flap was used to repair them. The animals were sacrificed at 2, 4 and 6 months after operation, respectively. The situation, and drawn with light microscopy, scanning electron microscopy. The results showed that repairing tracheal wall with sternocleidomastoid musculocutaneous flap has good supporting force, repairing airway lumen unobstructed, scarring of the wall and formation of granulation tissue. The periosteal surface covered with intact cilia columnar epithelium, deep New bone formation. Clinical treatment of 8 patients, including cicatricial tracheal stenosis in 5 cases, thyroid cancer invaded the tracheal wall in 2 cases, 1 case of tracheal papilloma malignant, were removed from the tracheal tube to resume normal respiratory function, follow-up efficacy and stability. Tip sternocleidomastoid muscle flap is the ideal laryngotracheal reconstruction material.
