目的探讨非高血压左心室肥厚(LVH)与血清胰岛素水平变化的关系。方法测定42例非高血压LVH患者的血清胰岛素、血糖和空腹血脂水平;用超声测定并计算心脏左室质量(LVM)和左室质量指数(LVMI),并按心脏超声测定分为非高血压LVH和左室正常组。结果LVH组的空腹血甘油三酯和胰岛素分别高于左室正常组,而胰岛素抵抗指数(HOMA法)高于左室正常组。直线相关分析显示非高血压LVH组HOMA指数与LVM、LVMI正相关(r=0.38、P<0.01及r= 0.29、P<0.01)。结论非高血压LVH患者的高胰岛素血症与LVH有密切相关,高胰岛素血症可能参与非高血压LVH的发生和发展。
Objective To investigate the relationship between non-hypertensive left ventricular hypertrophy (LVH) and serum insulin level. Methods Serum insulin, blood glucose and fasting blood lipid levels were measured in 42 patients with non-hypertensive LVH. The left ventricular mass (LVM) and left ventricular mass index (LVMI) were measured by ultrasound and divided into nonhypertensive LVH and normal left ventricle group. Results Fasting blood triglyceride and insulin in LVH group were higher than those in normal left ventricle group, while insulin resistance index (HOMA) was higher in LVH group than in normal LV group. The linear correlation analysis showed that the HOMA index of non-hypertensive LVH group was positively correlated with LVM and LVMI (r = 0.38, P <0.01 and r = 0.29, P <0.01). Conclusion Hyperinsulinemia in nonhypertensive patients with LVH is closely related to LVH. Hyperinsulinemia may be involved in the occurrence and development of nonhypertensive LVH.
