目的探讨通过神经松解减压对糖尿病周围神经感觉的影响。方法对30例糖尿病周围神经病人进行双下肢神经松解,在显微镜下分别在内踝管胫神经及其分支、膝外侧腓神经管腓总神经、足背腓深神经处在显微外科技术微创手术下分别减压神经受压及神经外膜、束膜切开松解。观察病人手术前及术后检查上述神经传导速度,足部麻木、疼痛症状缓解程度,两点辨别觉及10g尼龙单丝试验。结果术后患者麻木、疼痛症状明显缓解有效率分别达93.3%、96.67%,10g尼龙单丝试验阳性率达到93.33%,腓总神经、胫神经感觉神经传导速度明显加快(P<0.05),两点辨别觉明显改善(P<0.05)。结论糖尿病周围神经病经过在显微外科手术下进行神经管道性狭窄松解及神经外膜、束膜的松解后病人的麻木、疼痛症状明显缓解,神经感觉功能明显恢复。
Objective To investigate the effect of nerve decompression on peripheral nerve sensation in diabetic patients. Methods Thirty patients with diabetic peripheral neuropathy were subjected to bilateral lower extremity neurolysis. The tibial nerve of the medial malleolus and its branches under the microscope, the common peroneal nerve common peroneal nerve, the dorsal and pedicel deep peroneal nerve were treated by minimally invasive microsurgery Surgery were decompression nerve compression and epineurium, tunicotomy release. Observed the patient before and after surgery to check the nerve conduction velocity, numbness of the feet, the degree of pain relief, two-point discrimination and 10g nylon monofilament test. Results The postoperative numbness and pain relief rate were 93.3% and 96.67%, respectively. The positive rate of 10g nylon monofilament test was 93.33%, and the sensory nerve conduction velocity of the common peroneal nerve and tibial nerve was significantly increased (P <0.05). Two Discernment markedly improved (P <0.05). Conclusion After diabetic peripheral neuropathy underwent neurosurgical stenosis and neuromuscular detachment, the numbness and pain symptoms of patients with neuropathic pain were significantly relieved and neurological sensory function recovered obviously.
