马××,女,50岁。慢性风湿性心脏病联合瓣膜损害三年。两周来,心慌气短不能平卧。查体:端坐体位,口唇紫绀心界向两侧扩大,心率不齐,心尖区可闻及Ⅲ级收缩期及舒张期杂音,向腋下传导。主动脉瓣区可闻及Ⅲ级收缩期杂音,向颈部传导。两肺底可闻及湿性罗音。X线胸部照片提示:主动脉扩张:左心室扩大;肺瘀血。诊断:风湿性心脏病,二尖
