Acute renal failure can result from muscle damage caused by ischemia or direct trauma. Early identification of critically ill patients and aggressive treatment can prevent or reduce the extent of renal failure. The experience with previous treatment of whip injuries suggests that decreased venous blood bicarbonate concentration (VBC) is the most reliable predictor of renal failure. 64 cases of whiplash were prospectively studied to determine whether this hypothesis is correct and to clarify its basic pathophysiological processes. On admission, venous blood samples were taken for determination of urea, electrolytes, sugars and lactic acid, urine for occult blood test and sampling for spectral analysis. All patients received Ringer’s lactate solution intravenously to be admitted to hospital for 12 hours
