肝硬化患者的高氨血症一向被认为是大肠细菌尿素酶所产生。然而已知幽门螺旋菌(HP)因其很强的尿素酶活力(比尿素酶阳性肠菌大许多倍)而产生大量的氨。作者报告两例肝硬化病人,在清除HP后,其高氨血症得以明显减轻。现举例1为例:患者女,59岁,患丙型肝炎肝硬化6年。反复发生高氨血症和肝性脑病。经仔细治疗,意识改善,但空腹血氨仍高。酚红染料喷洒内镜检查。组织学和培养都证明HP在胃内弥漫性分布,乃予以根除HP治疗:
Hypernatremia in patients with cirrhosis has always been considered to be produced by the intestinal bacterial urease. However, Helicobacter pylori (HP) is known to produce large quantities of ammonia due to its strong urease activity, many times greater than that of urease-positive enteric bacteria. The authors report two cases of cirrhosis patients, clear HP, the hyperammonemia was significantly reduced. Example 1 is an example: a female patient, 59 years old, suffering from hepatitis C cirrhosis for 6 years. Hypernatremia and hepatic encephalopathy occur repeatedly.
