大的颅底种瘤切除产生的软组织、骨或硬脑膜损伤,需要游离组织转移修复重建。作者对7例颇底缺损的患者应用了改进的带血管脂肪游离腹直肌皮瓣转移行颅底重建术。其中4例硬脑膜缺损是肿瘤切除所致,仅1例应用游离筋膜移植,另3例因解剖结构复杂未修复硬膜。2例经前方和颅面联合入路,其中1例需半腭和眼眶的重建。全部病人均应用了去表皮腹直肌(DRAM)游离皮瓣成功闭合颅底缺损,无脑脊液漏,游离皮瓣均成活。术后有2例感染,
Large skull base tumor excision of soft tissue, bone or dura injury, the need for free tissue transfer repair reconstruction. The authors applied an improved flap-free flap with vascular fat free rectus abdominis skull base reconstruction in 7 cases of patients with substantial defect. Among them, 4 cases of dura mater defect were caused by tumor resection, only 1 case was treated with free fascia transplantation and the other 3 cases did not repair dura due to complicated anatomy. 2 cases by the front and craniofacial approach, of which 1 case required reconstruction of the half palate and orbit.
