目的探讨糖尿病周围神经病(DPN)患者的神经传导速度(NCV)改变及其价值。方法对60例糖尿病患者(观察组)使用丹迪Keypoin肌电诱发电位仪测定其双侧正中神经、尺神经和腓总神经的运动神经传导速度(MCV)和感觉神经传导速度(SCV),与30例健康人(对照组)进行对比分析。测定观察组患者空腹8 h后血糖,分析血糖与NCV的相关性。结果观察组尺神经、正中神经、腓总神经运动神经传导速度(MCV)分别为(49.17±4.93)、(44.92±4.6)、(40.29±3.62)m/s,对照组分别为(56.23±3.24)、(55.58±2.2)、(48.61±4.42)m/s,两组相比差异有统计学意义(均P<0.05);观察组尺神经、正中神经、腓总神经感觉神经传导速度(SCV)分别为(44.12±6.55)、(40.23±2.79)、(32.33±4.22)m/s,对照组分别为(51.10±4.49)、(52.03±5.20)、(47.11±5.13)m/s,两组相比差异有统计学意义(均P<0.05)。患者NCV与空腹血糖无明显关联,但与病程长短呈正相关。结论NCV检测有助于DPN的早期诊断,对临床诊断有指导意义。
Objective To investigate the changes of nerve conduction velocity (NCV) and its value in patients with diabetic peripheral neuropathy (DPN). Methods Motor neuron conduction velocity (MCV) and sensory nerve conduction velocity (SCV) of bilateral median nerve, ulnar nerve and common peroneal nerve were measured in 60 diabetic patients (observation group) by using Dandy Keypoin electromyographic evoked potentials 30 healthy people (control group) for comparative analysis. The fasting blood glucose was measured in the observation group after 8 h, and the correlation between blood glucose and NCV was analyzed. Results The motor nerve conduction velocity (MCV) of the ulnar nerve, median nerve and common peroneal nerve were (49.17 ± 4.93) and (44.92 ± 4.6) and (40.29 ± 3.62) m / s respectively in the observation group and 56.23 ± 3.24 ), (55.58 ± 2.2) and (48.61 ± 4.42) m / s respectively. There was significant difference between the two groups (all P <0.05). The sensory nerve conduction velocity of ulnar nerve, median nerve and common peroneal nerve ) Were (44.12 ± 6.55), (40.23 ± 2.79) and (32.33 ± 4.22) m / s in the control group, and were 51.10 ± 4.49, 52.03 ± 5.20 and 47.11 ± 5.13 m / There was significant difference between the two groups (all P <0.05). There was no significant correlation between NCV and fasting blood glucose, but positively correlated with the duration of the disease. Conclusion NCV test can be helpful for the early diagnosis of DPN and has clinical significance.
