背景:日本医科院校。目的:阐明皮内注射纯化蛋白衍生物(PPD)后对QuantiFERON-TB Gold(QFT G)试验的影响。设计:97名20~29岁的医科大学六年级学生同时进行QFT G和结核菌素皮内试验(TST)。第1次QFT-G阴性及TST直径<15mm的1个月后进行第2次QFT-G试验。结果:97名第1次QFT-G试验验者中有5名(5%)为阳性,33名进行第2次QFT-G试验,有 5名(15%)转为阳性,显示了QFT-G的增强现象。结论:皮内注射PPD后可能会增强QFT-G试验反应。该现象的诊断意义以及发生的免疫机制有待进一步研究。对于临床应用,特别是密切接触者筛选,需要用QFT-G评估时,我们要考虑到皮内注射PPD可能的影响。
Background: Japanese medical school. Objective: To elucidate the effect of intradermal injection of a purified protein derivative (PPD) on the QuantiFERON-TB Gold (QFT G) assay. DESIGN: Sixty-nine Grade 6 medical students from the age of 20 to 29 years performed both the QFT G and the tuberculin intradermal test (TST). The second QFT-G test was performed one month after the first QFT-G negative and TST diameter <15 mm. RESULTS: Five out of 97 (5%) QFT-G subjects were positive, 33 were in the second QFT-G test and 5 (15%) were positive, indicating that QFT- G enhancement phenomenon. CONCLUSIONS: The intradermal injection of PPD may enhance the QFT-G test response. The diagnostic significance of the phenomenon and the immune mechanism to be further studied. For clinical applications, especially for close contact screening, we need to consider the possible impact of intradermal injection of PPD when QFT-G evaluation is required.
