目的探讨MR氢质子波谱(1H-MRS)在乳腺肿瘤中的临床应用价值及其影响因素。方法回顾性分析均经手术病理证实的38例乳腺肿瘤患者的1H-MRS表现,并与病理结果比较,其中包括乳腺癌20例,良性肿瘤18例。判断1H-MRS诊断乳腺癌的价值。所有病例均于磁共振动态增强后行1H-MRS检查。结果20例乳腺癌11例出现胆碱(Choline,Cho)峰。18例良性肿瘤3例出现Cho峰。以Cho峰为恶性诊断标准,其诊断的敏感性为55.0%,特异性为83.3%,准确性为68.4%。结论1H-MRS对乳腺癌的诊断特异性较高,乳腺癌Cho峰检出率低的原因主要与病灶本身和技术等因素有关。
Objective To investigate the clinical value and influencing factors of MR hydrogen proton spectroscopy (1H-MRS) in breast tumors. Methods The 1H-MRS findings of 38 breast cancer patients confirmed by surgery and pathology were retrospectively analyzed and compared with the pathological findings including 20 cases of breast cancer and 18 cases of benign tumors. To determine the value of 1H-MRS in the diagnosis of breast cancer. All cases were dynamically enhanced 1H-MRS examination after magnetic resonance imaging. Results In 20 cases of breast cancer, 11 cases showed choline (Cho) peak. In 18 cases of benign tumors, Cho peak appeared in 3 cases. To Cho peak as a malignant diagnostic criteria, the diagnostic sensitivity was 55.0%, specificity was 83.3%, the accuracy was 68.4%. Conclusion The diagnostic specificity of 1H-MRS for breast cancer is high. The reason for the low detection rate of Cho peak in breast cancer is mainly related to the lesion itself and technology.
