目的探讨CDX2、p-p38在胃黏膜病变中的表达及其意义。方法采用免疫组织化学方法检测CDX2和p-p38在肠上皮化生和胃癌组织中的表达情况。结果CDX2在正常胃黏膜中无表达,在肠上皮化生、肠型胃癌、弥漫型胃癌组织阳性表达率分别为84.6%、57.9%、13.3%,肠型胃癌组织中CDX2水平显著低于肠上皮化生(P<0.05),肠型胃癌显著高于弥漫型胃癌(P<0.05);p-p38在正常胃黏膜中呈少量表达,在肠上皮化生、肠型胃癌、弥漫型胃癌组织阳性表达率分别为80.0%、15.8%、86.7%,肠型胃癌组织中p-p38水平显著低于肠化生(P<0.05),肠型胃癌显著低于弥漫型胃癌(P<0.05)。在肠化生中CDX2的表达与p-p38的表达呈正相关(P<0.05),而在肠型胃癌两者表达无显著相关性(P>0.05),弥漫型胃癌中两者表达也无显著相关性(P>0.05)。结论CDX2在肠上皮化生向胃癌尤其肠型胃癌发展过程中可能起到重要作用,而p-p38在肠化生形成中对CDX2表达可能起正向调节作用;p-p38在弥漫型胃癌中明显高表达可能与胃癌的发展有关。
Objective To investigate the expression and significance of CDX2 and p-p38 in gastric mucosal lesions. Methods Immunohistochemistry was used to detect the expression of CDX2 and p-p38 in intestinal metaplasia and gastric cancer tissues. Results CDX2 was not expressed in normal gastric mucosa. The positive rate of CDX2 expression in intestinal metaplasia, intestinal type gastric cancer and diffuse type gastric cancer was 84.6%, 57.9% and 13.3% respectively. The level of CDX2 in intestinal type gastric cancer was significantly lower than that in intestinal epithelium (P <0.05), intestinal type of gastric cancer was significantly higher than diffuse type gastric cancer (P <0.05); p-p38 in normal gastric mucosa showed a small amount of expression in intestinal metaplasia, intestinal gastric cancer, diffuse gastric cancer positive The expression of p-p38 in intestinal-type gastric cancer was significantly lower than that in intestinal metaplasia (P <0.05), and was significantly lower than that in diffuse-type gastric cancer (80.0%, 15.8%, 86.7%, respectively) There was a positive correlation between the expression of CDX2 and the expression of p-p38 in intestinal metaplasia (P <0.05), but no significant correlation between the expression of CDX2 in intestinal metaplasia (P> 0.05) and the expression of CDX2 in intestinal metaplasia Correlation (P> 0.05). Conclusion CDX2 may play an important role in the development of gastric cancer, especially intestinal type gastric cancer, while p-p38 may play a positive role in the expression of CDX2 in intestinal metaplasia. Significantly higher expression may be related to the development of gastric cancer.
