目的了解血脂综合指数在糖尿病以及冠心病中的分布水平,以便从新的角度对血脂各单项指标进行综合评价,更全面灵敏地反应血脂紊乱对糖尿病并发冠心病的影响,为临床建立综合评价指标提供依据。方法以血脂、血糖正常的健康体检者50例为A组,对209例住院患者年龄40~82岁。按疾病情况分为B组(DM组)、C组(CHD组)和D组(DM+CHD组)。分别测定血清总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、空腹血糖(FPG)等,并计算血脂综合指数(LCI)。结果B、C、D组TP、ALB水平与对照组比较,均有不同程度降低(P<0.05);各组TG水平升高,其中B组的TG升高有统计学意义(P<0.05);HDL~C水平有所降低(P<0.05),以B组最低,B、C组之间差异有统计学意义(P<0.05)。B、C、D组的LDL-C水平有增高趋势,但各组差异不明显(P>0.05)。B、D组TC/HDL-C、和LCI水平明显高于A、C组(P<0.05),B、D组之间差异无统计学意义(P>0.05)。结论血脂综合指数比单一的血脂指标更能较好地反映糖尿病合并冠心病的危险性,在临床上有一定的应用价值。
Objective To understand the distribution of lipids in diabetes mellitus and coronary heart disease in order to make a comprehensive evaluation of the individual indicators of lipids from a new perspective and more fully and sensitively reflect the effects of dyslipidemia on coronary heart disease with diabetes mellitus and provide a comprehensive evaluation index for clinical practice in accordance with. Methods A total of 50 healthy subjects with normal blood lipids and normal blood glucose were enrolled in group A and 209 hospitalized patients aged 40-82 years. Divided into group B according to the disease (DM group), C group (CHD group) and D group (DM + CHD group). Serum total cholesterol (TC), triglyceride (TG), high density lipoprotein (HDL-C), low density lipoprotein (LDL-C) and fasting plasma glucose (FPG) ). Results Compared with the control group, the levels of TP and ALB in groups B, C and D decreased (P <0.05), the levels of TG increased in each group, and the levels of TG in group B increased significantly (P <0.05) (P <0.05), the lowest in group B, the difference in group B and C was statistically significant (P <0.05). The levels of LDL-C in groups B, C and D showed an increasing trend, but there was no significant difference between groups (P> 0.05). The levels of TC / HDL-C and LCI in B and D groups were significantly higher than those in A and C groups (P <0.05). There was no significant difference between B and D groups (P> 0.05). Conclusion The comprehensive index of blood lipids better reflects the risk of diabetes mellitus complicated with coronary heart disease than the single lipid index and has some clinical value.
