目的分析血清胱抑素C与慢性心力衰竭的关系,并探讨血清胱抑素C评估心衰程度的价值。方法2015年3月——2016年10月选取本院59例慢性心力衰竭患者为研究对象,将其作为观察组,并选择同期住院且心功能正常的59例患者,将其作为对照组。收集2组患者的相关临床资料,并对其血清胱抑素C、尿酸、肌酐等生化指标进行检测。对比2组患者的血压、心率、血清胱抑素C、脑钠肽、左房室结构参数以及肾功能指标水平,并分析血清胱抑素C与慢性心力衰竭的关系及其评估心衰程度的价值。结果观察组血压、心率、血清胱抑素C、脑钠肽、左室舒张末内径、左房内径以及肾功能指标与对照组相比明显升高,其射血分数与对照组相比明显降低,差异有统计学意义(P<0.05);采用Pearson相关分析发现,观察组血清胱抑素C与脑钠肽、左房内径、肾功能指标呈正相关,与射血分数呈负相关(P<0.05);多元回归分析发现血清胱抑素C为脑钠肽、左房内径、射血分数的独立影响因素(P<0.05)。结论血清胱抑素C与慢性心力衰竭患者的心功能、心脏结构的关系密切,可作为评估心衰程度的重要临床指标。
Objective To analyze the relationship between serum cystatin C and chronic heart failure and evaluate the value of serum cystatin C in evaluating heart failure. Methods From March 2015 to October 2016, 59 patients with chronic heart failure in our hospital were selected as the observation group and 59 patients who were hospitalized in the same period with normal cardiac function were selected as the control group. The clinical data of two groups were collected, and the biochemical indexes such as serum cystatin C, uric acid and creatinine were detected. Blood pressure, heart rate, serum cystatin C, brain natriuretic peptide, left atrioventricular structural parameters and renal function parameters were compared between the two groups. The relationship between serum cystatin C and chronic heart failure and its assessment of heart failure value. Results Compared with the control group, the blood pressure, heart rate, serum cystatin C, brain natriuretic peptide, left ventricular end-diastolic diameter, left atrium diameter and renal function index in observation group were significantly higher than those in control group (P <0.05). Pearson correlation analysis showed that serum cystatin C in the observation group was positively correlated with brain natriuretic peptide, left atrium diameter and renal function, and negatively correlated with ejection fraction (P < 0.05). Multiple regression analysis showed that serum cystatin C was an independent influencing factor of brain natriuretic peptide, left atrium diameter and ejection fraction (P <0.05). Conclusions Serum cystatin C is closely related to the cardiac function and cardiac structure in patients with chronic heart failure and can be used as an important clinical index to evaluate the degree of heart failure.
